... Read moreHey future nurses! I know the PN NCLEX can feel overwhelming, but mastering those core nursing facts is key to success. I’ve been there, sifting through countless pages, trying to figure out what’s truly essential. That’s why I wanted to share some insights on those critical 'must-knows' that often appear on the exam and are vital for your practice, especially covering topics like the PERRLA assessment nursing concept. One of the first things you’ll learn and use constantly is the PERRLA assessment. It stands for Pupils Equal, Round, Reactive to Light, and Accommodation. It’s not just a fancy acronym; it's a quick, non-invasive way to assess neurological function and detect potential issues rapidly. From my experience, knowing how to properly perform a PERRLA pupil assessment nursing evaluation is fundamental. You'll check if both pupils are the same size (equal), their shape (round), how they constrict when a light is shone on them (reactive to light), and their ability to constrict when focusing on a near object and dilate when focusing on a far object (accommodation). Any deviation can signal anything from head trauma to drug effects, so always document your findings thoroughly! Beyond PERRLA, there are so many other nursing must knows that will serve you well. Think about patient safety and prioritization. I remember learning about the A, B, C, D, E hierarchy – Airway, Breathing, Circulation, Disability (neurological status, where PERRLA fits in!), and Exposure. Always tackle airway first! This foundational approach helps you prioritize in critical situations, which is exactly what the NCLEX tests. Another big one is fall prevention, especially for our elderly patients. Simple things like ensuring call bells are within reach, beds are in the lowest position, and understanding why falls happen (like cognitive impairment, polypharmacy, or environmental hazards) can make a huge difference. Medication administration is another area where precision is paramount. Mastering the different injection types – subcutaneous, intramuscular (IM), intradermal, and even the Z-track method – is non-negotiable. I found it helpful to visualize the layers of skin and muscle, and what angle each injection requires. For example, remembering to aspirate for most IM injections (though guidelines can vary, so always check your facility’s policy!) was something I constantly practiced. Don't forget the importance of effective communication. Dealing with hearing-impaired patients, for instance, requires patience and specific techniques like facing them directly, speaking clearly, and using visual aids. And of course, upholding patient rights is at the core of our profession. Knowing about informed consent, refusal of care, and confidentiality isn’t just for the exam; it’s about respecting human dignity. Finally, effective study habits really tie all these facts together. Instead of just rote memorization, try to understand the 'why' behind each fact. Why is this patient positioned this way? Why do we assess pain in a particular manner? Connecting the dots makes recall much easier under pressure. Using flashcards for quick facts and then applying them to case studies really helped me. Good luck with your studies – you’ve got this!
