It’s no secret that residents work A LOT of hours; however, the ACGME has implemented safeguards to ensure resident safety by limiting the number of hours residents put in a week. Certain specialties (often surgical specialties) are known to hit that 80hr work week on average max quite often, while others (like psychiatry for example) tend to average around 50-65 hour work weeks!
As a resident doctor, one of the most common questions I get asked is, "Do you really work 80 hours a week?" The short answer, as an image from my post suggests, is: sometimes, and sometimes even more! The Accreditation Council for Graduate Medical Education (ACGME) has indeed put in place crucial safeguards to limit resident duty hours, primarily for both patient safety and resident well-being. But understanding these ACGME hours rules and how they play out in real life is key. The cornerstone is the 80-hour work week average, calculated over a four-week period. This isn't a strict 80-hour cap every single week, which means some weeks can be significantly heavier than others, particularly during busy rotations or when covering for colleagues. I've definitely had weeks where I've pushed past that 80-hour mark, only to have lighter weeks balance it out later. This flexibility, while necessary, can lead to intense periods where you truly feel the grind. Then there's the critical 10-hour rest between shifts. This guideline is designed to ensure adequate rest and prevent fatigue-related errors. However, adherence can be challenging. Sometimes, patient care responsibilities, unexpected emergencies, or even the sheer volume of charting after a long shift can eat into that precious rest time. While programs strive to uphold this, the reality of medicine can sometimes make it a struggle. It's a constant battle to prioritize sleep when you're dedicated to your patients. Another significant rule is the 30-hour maximum shift with a mandatory rest day afterward. These extended shifts, often referred to as '24-hour calls' or 'long calls,' are incredibly demanding. You're constantly on, making critical decisions, often without much sleep. The immediate rest day following such a shift is absolutely essential for recovery, both physically and mentally. It's during these shifts that the true intensity of residency often becomes most apparent. Finally, the ACGME guidelines also ensure residents receive 4 days off monthly. This is crucial for preventing burnout and maintaining some semblance of a personal life. These days off are golden opportunities to recharge, see family and friends, or simply catch up on sleep and errands. Without these protected days, the demands of residency would be even more unsustainable. For instance, in surgical specialties, hitting the 80-hour average max is quite common, requiring careful scheduling to ensure these days off are maintained.
