Symptoms are multisystem by definition. If only one system is involved, MCAS is unlikely.

Ok, so your mast cells are everywhere in your body. They live in your skin. Gut. Blood vessels. Lungs. Brain. Connective tissue. Everywhere. Having MCAS means that your symptoms are in more than one of these systems at a time. It doesn’t mean you have to have symptoms in ALL of the different systems at the same time. It just definitely affects more than one system at a time.

So, let’s say someone has chronic hives and nothing else (no cardiovascular signs, no GI signs, no neurological signs etc) They’re more likely to have chronic spontaneous urticaria (or something similar along those lines). And will only need to address the hives.

With MCAS you’ll have multi system issues like flushing with tachycardia. Nausea with brain fog. Intestinal issues with lightheadedness. Or something like itching with blood pressure drop. Even subtle symptoms across different systems.

Keep in mind though, like I said before, it doesn’t mean it has to be every system at the same time. It also doesn’t mean they have to be severe symptoms in every system. Also, you won’t have identical symptoms in every flare. No two flares really look the same. And two different people with MCAS can and will have wildly different looking/feeling flares and symptoms.
