ECG calculations
When I first started learning ECG calculations, I found it challenging to remember all the different formulas and how to measure intervals correctly. However, using the box method as a visual aid really simplified the process for me. For example, calculating the heart rate for a regular rhythm by dividing 300 by the number of large boxes between R waves is straightforward once you get the hang of spotting those boxes on the ECG strip. For irregular rhythms, I recommend using the 6-second strip method. Counting the number of QRS complexes in six seconds and multiplying by 10 gives a reliable estimate of the heart rate. Measuring PR and QRS intervals can feel intimidating at first, but remembering that each small box represents 0.04 seconds and each large box 0.20 seconds makes it easier. For instance, multiplying the number of small boxes from the start of the P wave to the start of the QRS complex by 0.04 helps determine the PR interval. Knowing that normal PR intervals range from 0.12 to 0.20 seconds and QRS duration should be under 0.12 seconds helped me quickly identify abnormalities. I found it useful to keep a quick reference summary card with these key formulae and timing values handy during exams and clinicals. Practicing regularly by applying these calculations on different ECG strips builds confidence and sharpens interpretation skills.
