Moreover, unilateral crackles accompanied by fever and productive cough generally suggest pneumonia, where supplemental oxygen supports respiratory function effectively. One of the most urgent lung sounds to identify is absent or decreased breath sounds on one side with tracheal deviation and hypotension, often signaling a tension pneumothorax. Immediate needle decompression is paramount in these cases to prevent further respiratory compromise. Understanding the subtle differences between fine and coarse crackles, or between wheezes and rhonchi, helps EMS providers tailor their treatment plans instead of relying on general approaches. For example, crackles that sound like rolling hair between fingers indicate alveolar fluid whereas coarser crackles hint at mucus in larger airways, suggestive of different underlying pathologies. Mastering lung sound assessment not only aids in diagnosing but also in monitoring treatment response during transport. While technology assists with imaging and diagnostics, expert auscultation skills remain invaluable in pre-hospital care.
