フロセミドは利尿作用が強力で、使用する際には尿量や血圧測定だけでなく、もう一歩踏み込んだ観察が必要です。私自身の経験でも、患者さんの低カリウム血症を見逃さずに早期発見できたことで、重篤な不整脈の発生を防ぐことができました。フロセミドは尿と一緒にカリウムも排出してしまうため、定期的な血液検査や心電図モニターでSTの低下やT波の変化を確認することが大切です。 また、脱水症状のチェックも見落とせません。口渇感、皮膚の乾燥、ふらつきが出ていないかこまめに観察し、不安定な血圧の変動も注意深く見ることで、体液バランスの乱れを早期に察知可能です。さらに、フロセミドは耳への影響もあるため、患者さんが耳鳴りや難聴を訴えた際には速やかに報告し、必要に応じて医師と連携を取りましょう。 看護現場では忙しさから基本的な観察項目に偏りがちですが、こうした副作用の兆候を多角的に見ることで患者さんの安全確保に繋がります。スキマ時間で学べる知識として、これらのポイントを日々の観察リストに加えてみてはいかがでしょうか。副作用の早 期発見と対応で患者さんのQOL向上に貢献できることを実感しています。
