「要介護5で施設に入ると費用はいくら？」は、家族会議で一番揉めやすいテーマだと感じました。結論から言うと、月額は“どの施設を選ぶか”で大きく変わります。同じ要介護5でも、介護サービス費そのものに加えて、食費・居住費・日用品などの「介護保険の外のお金」が積み上がるからです。 まず押さえたいのが施設のタイプ別イメージです。特別養護老人ホーム（特養）は公的色が強く、費用が比較的抑えられやすい一方で、待機が出やすいのが現実。介護老人保健施設（老健）は“在宅復帰を目指す中間施設”の位置づけで、長期入所が前提ではない点に注意が必要でした。有料老人ホームは設備や人員体制、立地で価格差が大きく、月額が読みづらいので、見学時に「月額に含まれる範囲」を必ず確認するのがおすすめです。 次に、見落としがちな費用項目をチェックすると安心です。私が実際に比較して驚いたのは、介護保険の自己負担以外に、食費・居住費（部屋代）、オムツ代や日用品、理美容、医療費、嗜好品などが発生すること。施設によってはオムツが定額に含まれる／実費請求になるなど運用が違い、毎月のブレに直結します。「月額いくらですか？」だけでなく、「追加料金が出るのは何ですか？」を質問リストに入れると失敗が減りました。 自己負担を抑える視点では、“世帯分離”が話題に上がりやすいです。うまく当てはまると、介護費の自己負担や食費・居住費の負担が軽くなる可能性があります。ただし、国民健康保険料が上がるなど逆効果になるケースもあるので、我が家は市区町村窓口やケアマネさんに「世帯分離をした場合の試算」を相談して判断しました。ネット情報だけで決めるのは危険だと痛感しています。 最後に、要介護5の施設費用を考えるときの現実的な進め方です。 1）候補施設ごとに「月額の内訳表」をもらう（介護サービス費／食費／居住費／その他） 2）要介護5で想定される追加（オムツ、医療連携、看取り対応）を確認 3）公的制度（負担軽減、世帯分離の可否）を窓口で試算 この3ステップだけでも、家族の不安とすれ違いがかなり減りました。
