褥瘡の治療に携わる中で、軟膏や薬剤の選択はとても難しく感じることが多いと思います。私も看護師として勤務していた際、傷の状態が変化するたびに適切な薬剤を判断するのに大変苦労しました。まず、褥瘡の治療薬には乾燥した傷に有効な白色ワセリンや、炎症や痛みを和らげるアズノールなど、さまざまなタイプがあります。これらはそれぞれ傷の段階や症状に応じて使い分けることが必要です。 特に、黒く壊死した組織にはプロメラインスプレーが効果的で、タンパク分解酵素の働きで壊死部分を除去しやすくします。また、ヨウ素を含む薬剤は殺菌効果が高い一方、アレルギー体質の方には禁忌となるため注意が必要です。浸出液が多い傷には高い吸水力を持つユーバスタを活用することで、清潔な環境を維持しやすくなりました。 仕事中は忙しく、資料をその場で調べる余裕がないことも多いですが、早見表やまとめ資料をバインダーに挟んで手軽に確認できるように準備しておくと非常に便利です。自分の経験から言うと、こうした資料を常備しておくことで薬剤選択に自信がつき、患者さんへのケアの質も向上しました。 さらに、傷の変化をしっかり観察し、異変があれば早めに医師へ報告することが重要です。看護師自身が薬剤に関する基礎知識を持つことで、よりスムーズなコミュニケーションが可能になり、患者さんに最適な治療につなげやすくなります。これから褥瘡ケアに関わる方は、ぜひ今回の早見表を活用し、日々の観察とケアに役立てていただきたいと思います。
