Read moreจากที่ได้ไปฝึกงานพยาบาลห้องฉุกเฉิน (ER) รอบกลางคืน เรารู้สึกว่า “จังหวะงาน” ต่างจากช่วงกลางวันพอสมควรเลยค่ะ กลางคืนคนอาจดูน้อยลงเป็นช่วงๆ แต่พอมีเคสเข้ามาทีคือเข้ามาพร้อมกันหลายเคส ทำให้ต้องโฟกัสและทำงานเป็นทีมมากๆ สิ่งที่เราได้เรียนรู้เยอะมากคือการตั้งสติและฟังคำสั่งให้ชัด เพราะในห้องฉุกเฉินเวลามีความหมายจริงๆ อีกอย่างที่อยากเล่าคือเรื่องการคัดแยกผู้ป่วย หรือที่เห็นในเอกสาร/ป้ายว่า Levels Triage ตอนแรกเราไม่ค่อยเข้าใจว่าทำไมต้องแบ่งระดับละเอียด แต่พอเจอสถานการณ์จริงจะเห็นเลยว่าช่วยจัดลำดับความเร่งด่วนได้มาก เช่น ผู้ป่วยอาการหนักมากต้องได้พบทีมแพทย์ก่อน ส่วนเคสที่อาการคงที่อาจต้องรอคิวเพื่อให้ทรัพยากรในห้องฉุกเฉินถูกใช้กับคนที่จำเป็นที่สุด เราได้ฝึกสังเกตอาการเบื้องต้นจากการดูสีหน้า การหายใจ การตอบสนอง และเรียนรู้ว่าข้อมูลเล็กๆที่ถามเพิ่มสามารถเปลี่ยนระดับความเร่งด่วนได้ ด้านทักษะปฐมพยาบาลเบื้องต้นที่ได้จับงานจริงๆ (ภายใต้การดูแลของพี่ๆพยาบาล) คือการเตรียมอุปกรณ์ให้พร้อมก่อนเริ่มงานทุกครั้ง เช่น ใส่ถุงมือให้ถูกต้อง จัดพื้นที่ทำงานให้ปลอดภัย และเช็กของใช้จำเป็นใกล้มือ เพราะบางทีต้องช่วยหยิบ/ส่งอุปกรณ์อย่างรวดเร็ว เราได้เห็นการทำงานใกล้อุปกรณ์ทางการแพทย์มากขึ้น ทำให้รู้ว่าความสะอาดและขั้นตอนป้องกันการติดเชื้อสำคัญมากๆ โดยเฉพาะใน ER ที่เจอผู้ป่วยหลากหลาย หลายคนอาจสงสัยเรื่อง “นอนในห้องฉุกเฉิน” หรือการอยู่เวรกลางคืนจริงๆ บอกตรงๆว่าเหนื่อย แต่เป็นความเหนื่อยที่ทำให้เราโตขึ้นค่ะ ช่วงพักจะเป็นช่วงสั้นๆ และต้องพร้อมกลับไปช่วยงานได้เสมอ เราแนะนำว่าถ้าใครต้องฝึกเวรกลางคืน ให้เตรียมตัวเรื่องการนอนก่อนเข้าเวร (งีบให้พอ), พกน้ำ/ของกินเล็กๆที่ช่วยเติมพลัง และใส่รองเท้าที่เดินสบาย เพราะเดินเยอะมาก สิ่งที่ประทับใจที่สุดคือพี่ๆพยาบาลห้องฉุกเฉินใจดีและสอนงานละเอียดมากค่ะ เวลาทำอะไรไม่มั่นใจให้ถามทันที ไม่ต้องกลัวโดนว่า เพราะความปลอดภัยของผู้ป่วยมาก่อนเสมอ วันไหนงานหนักๆ พี่ๆยังให้กำลังใจและดูแลกันดีมาก บางวันมีน้ำชาเขียวเป็นของขวัญก่อนกลับบ้านด้วย รู้สึกว่าเล็กๆน้อยๆแบบนี้ช่วยฮีลใจสุดๆ ถ้าใครเป็นนักศึกษาพยาบาลฝึกงานโรงพยาบาลและกำลังจะขึ้น ER เราว่าสิ่งที่ช่วยได้มากคือจดโน้ตคำศัพท์/ขั้นตอนที่เจอบ่อย ฝึกสื่อสารให้กระชับ และตั้งเป้าว่าวันนี้จะเรียนรู้อะไรเพิ่ม 1 อย่าง แค่นี้ก็ทำให้การฝึกงานพยาบาลห้องฉุกเฉินมีคุณค่ามากขึ้นในทุกกะเลยค่ะ
