Hey fellow phlebotomists and medical students! I remember when I was first starting out, the sheer number of different blood collection tubes and their specific uses felt overwhelming. One question that often came up, and for good reason, was about the red tubes that can be used as a discard tube, and what they often contain. From my personal experience in phlebotomy, understanding the role of the plain red top tube as a discard is absolutely critical for accurate lab results. These tubes are usually made of glass or plastic and typically contain no additive — they're designed for serum collection, often used for blood banking, toxicology, or specific chemistry tests where a clot is desired. But their 'discard' role is where things get really interesting and important. So, why do we use a plain red top tube as a discard? The primary reason is to prevent additive carryover. Imagine you’re drawing blood and the first tube in your order of draw has an anticoagulant, like EDTA from a lavender top. If you then draw a tube for coagulation studies (like a blue top) directly after, residual anticoagulant from the previous tube could contaminate your sample and skew the results, leading to incorrect diagnoses or treatments. By drawing a discard tube first, you clear any potential contaminants from the needle and tubing, ensuring the subsequent tubes receive a pure, uncontaminated blood sample from the patient's VESSELS. Another key reason, especially when using a butterfly needle, is to clear the air. The tubing of a butterfly needle contains a small amount of air. If the very first tube drawn is for coagulation studies (like a light blue top, which requires a precise blood-to-additive ratio), that air can compromise the fill volume and, consequently, the accuracy of the test. A discard tube ensures the entire line is filled with BLOOD Structure — a mix of Plasma (55%), erythrocytes, whitebloodc, and platelets — before the critical samples are collected. What does a discard red top actually *contain*? When used as a discard, it simply collects the initial flow of blood, effectively acting as a 'rinse' for the line. It's not usually processed for diagnostic purposes itself. The blood within it will clot, as there's no anticoagulant. The goal isn't to analyze the contents of the discard tube but to ensure the integrity of the next tubes in the sequence. I always tell my students: think of it as sacrificing one tube to save all the others from potential inaccuracy. It's a small step that makes a huge difference in patient care and reliable diagnostics. Always check your facility's specific order of draw, but generally, a plain red top or another non-additive tube is the go-to choice for a discard tube when needed.
