Pediatric Endocrine Disorders & Must Know!🩺👶
The endocrine system plays a HUGE role in growth, metabolism, and hormone regulation in kids! Here are the key disorders, signs, and management tips you need to know for Peds Endocrine 🏥📚
⸻
🛑 1. Type 1 Diabetes (T1D) - “Insulin-Dependent Diabetes”
✅ Cause: Autoimmune destruction of pancreatic beta cells → No insulin production.
✅ S/S: 3 P’s → Polyuria, Polydipsia, Polyphagia, weight loss, fatigue.
✅ Management:
• Insulin therapy (lifelong)
• Monitor blood glucose regularly
• Carb counting + exercise for glucose control
• DKA Risk 🚨 → Fruity breath, Kussmaul respirations, dehydration!
⸻
🦋 2. Congenital Hypothyroidism (Cretinism)
✅ Cause: Thyroid hormone deficiency at birth → Can lead to intellectual disability if untreated.
✅ S/S: “Everything is slow!”
• Poor feeding, lethargy, hoarse cry, constipation
• Puffy face, large tongue, hypotonia
✅ Management:
• Lifelong Levothyroxine (Synthroid)
• Early treatment prevents cognitive delays!
⸻
🔥 3. Hyperthyroidism (Graves’ Disease)
✅ Cause: Overactive thyroid (autoimmune) → Too much T3/T4.
✅ S/S: “Everything speeds up!”
• Tachycardia, weight loss, heat intolerance, sweating
• Exophthalmos (bulging eyes) 👀
✅ Management:
• Methimazole or PTU (antithyroid meds)
• Beta-blockers (propranolol) for symptom control
• Radioactive iodine or surgery (severe cases)
⸻
📏 4. Growth Hormone Deficiency (Pituitary Dwarfism)
✅ Cause: Low GH → Short stature, delayed growth.
✅ S/S:
• Below 3rd percentile height for age
• Delayed puberty, increased fat deposits
✅ Management:
• Daily GH injections (Somatropin)
• Early treatment = best height potential!
⸻
🧂 5. Congenital Adrenal Hyperplasia (CAH)
✅ Cause: Adrenal glands can’t produce cortisol → Leads to excess androgens (male hormones).
✅ S/S:
• Girls: Ambiguous genitalia at birth 🚼
• Boys: Early puberty (precocious puberty)
• Low BP, dehydration (Salt-wasting crisis)
✅ Management:
• Lifelong corticosteroids (Hydrocortisone, Fludrocortisone)
• Monitor electrolytes & hydration
⸻
🩸 6. Diabetes Insipidus (DI) - “Water-Loss Disorder”
✅ Cause: Lack of ADH (vasopressin) → Kidneys can’t retain water.
✅ S/S:
• Excessive thirst & urination (Polyuria, Polydipsia)
• Dehydration risk 🚨
• Low urine osmolality, high serum sodium
✅ Management:
• Desmopressin (DDAVP) = synthetic ADH
• Increase fluid intake
⸻
📌 Quick Mnemonics to Remember!
💉 T1D: “Hot & Dry = Sugar High” (Hyperglycemia) vs. “Cold & Clammy = Need Some Candy” (Hypoglycemia)
🐌 Hypothyroidism: “Everything slows down” (fatigue, constipation, bradycardia)
🚀 Hyperthyroidism: “Everything speeds up” (weight loss, tachycardia, heat intolerance)
