Principles of follow-up examination after lung cancer surgery
We usually need to do CT to see if there is a recurrence. It is usually done every 4-6 months after 2 years of surgery. After that, CT is recommended per year.
Here, pulmonary CT examination should inject color or not inject. There are pros and cons. How? Let's see.
1️⃣ CT Non-Contrast CT
Suitable when
• Follow-up routine (routine follow-up)
• Often suitable for screening for lung cancer
• See postoperative fibrosis / scar
• The patient is allergic to radiation or has kidney degeneration.
Advantage
• Short time. No need to refrain from food.
• Safer
• Less time, lower cost
• No effect on kidneys
✅ is usually only seen in lung texture, usually used to track where the lungs or lung tissue do not see lymph nodes.
2️⃣ CT Color Injection (Contrast-Enhanced CT)
Suitable when
• Suspected lymph nodes
• Split surgical lump vs repeated cancer lump
• Evaluate pleura / thoracic wall / blood vessels
• Plan additional surgery or treatment
Look at the progression of intravenous nodules.
Restrictions
• Radiation allergy risk
• Be careful in patients with kidney degeneration... If kidney degeneration, consult a kidney doctor before doing it.
• You must abstain from food for 8 hours.
⏱ Popular frequency (estimated)
• Years 1-2: Every 6 months after surgery
• After that: Once a year
(May adjust according to disease stage and cancer type)
Personally, in my work...
Lung cancer is often seen in old patients, who often have kidney degeneration, and I rarely inject color because patients have a lot of side conditions...
The limitation is that we can only see lung texture and local regional recurrence.
But anyway, in the first case... before lung cancer surgery, we recommend color injection in every case, if not the GGN group (very small chance of distribution).
