I've been there, staring at my "Capillary Samples" notes, wondering if I'll ever get it all straight for the national exam. But trust me, once you understand the 'why' behind each step, it clicks! One of the most frequently asked questions and a common point of confusion is, "Why do we wipe away the first drop of blood during a capillary puncture?" It's not just a random step; it's vital for accurate results. That first drop contains a mix of tissue fluid (interstitial fluid) and cellular debris from the puncture site. If we collected this fluid, it could dilute the sample, introduce contaminants, and interfere with certain test results, like clotting studies or electrolyte levels. Wiping it away ensures you're collecting a pure sample of capillary blood, giving a truer representation of the patient's physiological state. Think of it as clearing the path for the good stuff! Now, let's talk about ensuring adequate blood is collected, especially when dealing with critical tests like oxygen levels (blood gases). For these, you often need an "arterialized" capillary sample, meaning the blood should closely resemble arterial blood. To achieve this, pre-warming the puncture site (e.g., with a warm compress for 3-5 minutes) is key. This increases blood flow, making collection easier and improving the arterialization of the sample. When performing the puncture, ensure it's a quick, decisive stick with the proper lancet depth. After wiping the first drop, allow a large, free-flowing drop to form naturally. *Crucially, avoid "milking" or excessively squeezing the finger/heel*, as this can introduce more tissue fluid and cause hemolysis (breakdown of red blood cells), rendering the sample unsuitable for testing, particularly for blood gases. When collecting into a capillary tube, ensure the blood flows smoothly into the tube by capillary action, holding the tube horizontally or slightly downward to avoid air bubbles and allow gravity to assist, but primarily letting the capillary action do its job. For blood gas analysis, it's essential to collect the sample quickly and completely fill the specific capillary tube designed for it, sealing it immediately to prevent air exposure. Another critical aspect for "Capillary Samples" is the order of draw, which, as my "Phlebotomy Notes" emphasize, is different from venipuncture. For capillary collections, we follow the "EHAS" rule to help us remember: EDTA (Lavender/Pink top): Collected first. EDTA prevents platelet clumping, which is essential for accurate platelet counts, and it's the additive most sensitive to clumping. Heparin (Green top): Collected second. Heparin is an anticoagulant used for various chemistry tests. Additives (Other tubes with additives, like Sodium Fluoride for glucose): Collected third. Serum (Red/Gold/Tiger top, no additive or clot activator): Collected last. This specific "EHAS order of draw" helps prevent contamination of additive-free tubes with anticoagulants and ensures accurate results, especially for tests where platelet integrity is crucial. Remember, techniques for "heel stick procedures" in infants for "infant screening tests" follow similar principles but require even greater care due to the delicate nature of infant skin and their small blood volume. Always ensure the heel is warmed, the puncture is superficial, and the collection is free-flowing, adhering to the EHAS order to safeguard the integrity of these vital screening tests. Mastering these techniques takes practice and a solid understanding of the 'why'. Keep studying your "phlebotomy study materials," visualize the steps, and understand the impact of each action on the patient and the test results. You've got this!

Hello all! I am a phlebotomist of 21 years. I just wanted to add a little side note on your heel/big toe sticks on babies. If the child is walking, you shouldn’t stick their feet, also when collecting blood whether from a foot or finger stick, it’s best to let the baby move and cry, it helps the blood flow better. ❤️
