When a Medication Error Almost Hurts A Patient

I’ll never forget the night I almost caused patient harm. I was giving meds on a med-surg floor during a crazy night shift. One of my post-op patients needed pain medication, and in the rush, I reached for the wrong vial, almost giving a dose that was double what they actually needed. I paused at the last second, checked the MAR, and realized my mistake. My heart was racing. I took a deep breath, explained to my charge nurse, and corrected it before anything happened. That moment shook me. It reminded me that even small distractions, fatigue, or pressure can have big consequences. From that night on, I never skip a double-check, never assume, and always scan, even when I feel confident. as a patient I always demand they draw the medication from the vial infront of me reading it and announcing the amount or it's a big fat no.
