One of my post-op patients needed pain medication, and in the rush, I reached for the wrong vial, almost giving a dose that was double what they actually needed. I paused at the last second, checked the MAR, and realized my mistake. My heart was racing. I took a deep breath, explained to my charge nurse, and corrected it before anything happened. That moment shook me. It reminded me that even small distractions, fatigue, or pressure can have big consequences.
