POTS | Anemia | Low Iron Stores 🩸

Low energy, dizziness, headaches... sound familiar?

For many with POTS, heavy periods, or chronic fatigue, the problem isn't always anemia, it's low iron stores, and that can drain you just as much.

This post breaks down:

4 Iron stores vs anemia

• Heme vs non-heme iron

Why vitamin C matters

* The gentler supplements that don't wreck your stomach

If you've ever been told your labs look "fine" but you still feel awful, this one's for you.

... Read morePostural Orthostatic Tachycardia Syndrome (POTS) often presents symptoms similar to iron deficiency such as fatigue, dizziness, and brain fog. However, many with POTS are not truly anemic but suffer from low iron stores, which can significantly exacerbate their symptoms. Unlike anemia, defined by low hemoglobin levels impairing oxygen transport, low iron stores mean ferritin levels are depleted even if hemoglobin remains within normal ranges. This nuance is critical because normal lab results might mask underlying deficiencies that impact patient wellness. For individuals with heavy menstrual bleeding or chronic fatigue, iron loss may outpace replenishment, reducing ferritin well below functional thresholds. While typical medical ranges consider ferritin levels of 12-15 ng/mL as normal, many patients with POTS only feel symptom relief when ferritin rises above 50 ng/mL. This indicates the importance of re-examining lab results in the context of functional health rather than only standard reference intervals. Iron comes in two forms: heme iron from animal sources, which is more readily absorbed, and non-heme iron found in plant foods and many supplements, which absorbs less efficiently. Vitamin C plays a vital role in enhancing non-heme iron absorption by converting it into a more bioavailable form. Conversely, substances like coffee, tea, and calcium can inhibit iron uptake; spacing these around iron intake is advisable. Gentle iron supplements such as iron bisglycinate, gluconate, or fumarate are preferred alternatives to ferrous sulfate, as they minimize gastrointestinal side effects like constipation and headaches. Regular monitoring of hemoglobin, ferritin, iron saturation, B12, and folate every six months is recommended, especially if symptoms persist despite 'normal' labs. Ultimately, patients must advocate strongly when told their labs look fine but they still experience debilitating symptoms. Deeper testing and comprehensive treatment addressing iron status, sleep quality, and overall nutrition contribute to improved quality of life. Prioritizing early bedtimes and consistent iron monitoring can reduce POTS flares and support heart function under stress. Understanding these interrelated factors empowers informed discussions with healthcare providers, enabling tailored approaches that address root causes of fatigue and iron deficiency beyond traditional anemia diagnosis.
