🌬️ Respiratory Assessment

Your first line of defense is a solid assessment!

👂 What to look for:

• Rate: Normal (12–20 bpm). Too fast = Tachypnea, Too slow = Bradypnea.

• Effort: Use of accessory muscles, nasal flaring, or tripod positioning.

• Sounds: Crackles = fluid, Wheezes = narrowed airways, Stridor = obstruction.

• O2 Saturation: Should be ≥ 95% unless chronic respiratory conditions are present.

✨ Pro Tip: Always start with inspection, then palpate, percuss, and auscultate!

🩸 Arterial Blood Gases (ABGs)

Your key to understanding acid-base imbalances!

• pH: 7.35–7.45

• PaCO2: 35–45 mmHg

• HCO3: 22–26 mEq/L

✨ Quick Guide:

1️⃣ Low pH + High PaCO2 = Respiratory Acidosis (e.g., COPD, hypoventilation).

2️⃣ High pH + Low PaCO2 = Respiratory Alkalosis (e.g., anxiety, hyperventilation).

3️⃣ Low pH + Low HCO3 = Metabolic Acidosis (e.g., DKA, renal failure).

4️⃣ High pH + High HCO3 = Metabolic Alkalosis (e.g., vomiting, antacid overuse).

💡 Pro Tip: Use the ROME method—Respiratory Opposite, Metabolic Equal—to interpret imbalances!

🚨 Acute Respiratory Distress Syndrome (ARDS)

A life-threatening condition where the lungs can’t oxygenate properly.

🛑 Causes: Sepsis, trauma, aspiration, pneumonia.

🛑 Symptoms: Severe hypoxemia that doesn’t improve with oxygen, crackles, tachypnea.

✨ Nursing Priorities:

• Mechanical ventilation with PEEP (Positive End-Expiratory Pressure).

• Frequent ABG monitoring.

• Positioning: Prone helps improve oxygenation.

💨 Chest Tubes

Chest tubes = Lifesaving for pneumothorax, hemothorax, or pleural effusion.

✨ Key Nursing Interventions:

1️⃣ Monitor Drainage: Report >100 mL/hr or bright red output.

2️⃣ Assess for Air Leaks: Continuous bubbling in the water seal chamber = leak!

3️⃣ Keep Emergency Equipment Nearby: Sterile gauze, clamps, and a chest tube system.

4️⃣ Patient Positioning: Semi-Fowler’s to promote drainage and lung expansion.

💡 Pro Tip: If the chest tube becomes dislodged, cover the site with sterile gauze and call the provider immediately.

✨ Nursing Intervention Focus

Always prioritize oxygenation and ventilation:

• Sit patients upright.

• Use incentive spirometry.

• Monitor ABGs and respiratory status closely.
