Mild cases may be managed conservatively with supplemental oxygen and close monitoring. However, larger pneumothoraces often require chest tube insertion to re-expand the lung and relieve pressure. Spontaneous pneumothorax generally affects young, thin males or individuals with underlying lung diseases such as chronic obstructive pulmonary disease (COPD). It occurs without obvious trauma and presents with sudden pleuritic chest pain, cough, and breathing difficulty. Asymmetrical chest expansion and hyperresonance on percussion are classic examination findings. Management varies from observation with oxygen to chest drainage depending on size and patient stability. Open pneumothorax results from penetrating chest injuries that allow air to enter the thoracic cavity, causing severe respiratory distress. A characteristic "sucking" sound may be heard at the wound site, and bubbles of blood or subcutaneous emphysema may be visible around the trauma. Immediate steps include placing the patient in a high Fowler's position, covering the wound with a three-sided occlusive dressing to prevent air entry on inspiration, and preparing for urgent chest tube placement in the 5th or 6th intercostal space midaxillary line to stabilize intrathoracic pressure. In all types, monitoring vital signs and oxygen saturation is crucial to detect deterioration. Pain control and establishing intravenous access are important for supportive care. Rapid recognition and intervention can prevent complications such as tension pneumothorax and respiratory failure. Understanding these three types equips nursing students, healthcare professionals, and caregivers with essential knowledge to provide timely and effective treatment for pneumothorax.
