5 Questions I Ask During Handoff
During my nursing career, I've found that asking the right questions during handoff is key to delivering safe, high-quality care. One question I always prioritize is: "What's the one thing that will hurt this patient the fastest if it goes wrong?" This sharp focus helps me identify critical risks early. I also make it a point to ask about any subtle changes since the last shift—even if they seem resolved. This can include shifts in vital signs, medication adjustments, or new symptoms. Knowing what’s normal for each patient is vital too, as it helps tailor care and detect anomalies promptly. Another helpful practice is to inquire about what concerns my colleagues the most, even if it’s just a small worry. This often uncovers early warning signs or potential complications that might not yet be evident. Lastly, I confirm what tasks or treatments haven’t been completed. This prevents important interventions from being overlooked and reduces the chance of adverse events. Incorporating these five questions into handoff routines not only strengthens communication among nursing staff but ultimately improves patient safety and care continuity.
