Recognizing the stages of pressure injuries—from Stage 1, where the skin is reddened but intact, to Stage 4, involving full thickness tissue loss with exposed bone or muscle—helps in determining the urgency of interventions and the proper wound care protocols. One useful tip is to remember that "unstageable" pressure injuries have tissue damage obscured by slough or eschar, meaning you need to carefully assess after debridement to reveal the true extent of the wound. Also, deep tissue injuries can appear as purple or dark discoloration even if the skin surface seems intact, which may initially be overlooked. Equally important is understanding wound drainage types, as they provide clues to the wound's healing phase or possible infection. For example, serous drainage—is clear and watery, indicating healthy healing—while purulent drainage is thick, yellow or green, and often has a foul odor, signaling infection requiring immediate attention. Recognizing sanguineous (bright red) or serosanguineous (pinkish) drainage helps differentiate normal post-injury bleeding versus potential complications. In my clinical experience, noting the amount of drainage also guides dressing changes: minimal drainage means dressing may last longer while heavy drainage requires frequent changes to prevent skin maceration and infection. Knowing these distinctions improved my efficiency during patient rounds and boosted my confidence during exams. For anyone preparing for nursing exams or working in wound care, I highly recommend making simple charts or flashcards for pressure injury stages and drainage types. These become invaluable quick references during busy shifts. Overall, mastering these concepts enhances patient outcomes by facilitating early identification and appropriate treatment of wounds.
