- looking at future upgrades and patching only doing them when the system fails or the vendor has made a request to update

- failing to improve the system after several years and expecting it to just continue until the system gives up

- failing to forecast operational costs after the project development is done

- failing to put specific monitoring and observability processes in place when something fails
