Understanding aphasia is essential for neuro-diagnostic success. Broca aphasia (non-fluent, effortful speech) and Wernicke aphasia (fluent but meaningless "word salad") both present with impaired repetition due to cortical damage. When repetition is preserved, the deficit is classified as Transcortical, typically caused by watershed zone infarcts that spare the core language circuit. Conduction aphasia is the "one-deficit" subtype where only repetition is impaired because of damage to the arcuate fasciculus bridge. Finally, Global aphasia represents the most severe form, where Broca, Wernicke, and the connecting fibers are all infarcted, resulting in a total loss of comprehension and expression. For example, Broca aphasia patients often exhibit broken, effortful speech but retain comprehension and are visibly frustrated by their speech difficulties. In contrast, Wernicke aphasia presents as fluent, faster speech that lacks meaning — often termed 'word salad' — with impaired comprehension and unawareness of the deficit. I found that conduction aphasia stands out due to its hallmark feature: impaired repetition with otherwise preserved fluency and comprehension. This is explained by damage to the arcuate fasciculus, the critical white matter tract connecting Broca’s and Wernicke’s areas. Transcortical aphasias, caused by watershed infarcts, spare the core language areas and thus preserve repetition, which can be a key diagnostic clue. Global aphasia, on the other hand, involves extensive damage to Broca’s, Wernicke’s, and connecting fibers, leading to severe deficits in both comprehension and expression — often resulting from widespread middle cerebral artery strokes. From a practical standpoint, using a simple flowchart that assesses fluency first, then comprehension, and finally repetition can drastically improve diagnostic speed and accuracy. I recommend also testing repetition with complex phrases like "No ifs, ands, or buts" as conduction aphasia patients struggle uniquely with this. Understanding the neurological basis behind these aphasia types enriches clinical assessments. Knowing which MCA branches supply the arcuate fasciculus or the frontal and temporal language areas helps localize lesions on imaging studies. This knowledge is invaluable in guiding patient management and rehabilitation plans. Overall, integrating the fluency-comprehension-repetition framework with anatomical correlations enhances both learning and clinical practice when working with stroke patients or anyone presenting with aphasia.
