Insulins You Should Know as a Pharmacy Tech 💉🧊

When I first stepped into the pharmacy world, I didn’t realize how many different types of insulin there were and more importantly, how many patients didn’t fully understand what they were taking!

Every day, I meet people who are prescribed insulin but aren’t sure what type they’re on, how fast it works, or even how to store it properly. That’s where we come in — not just to count and label, but to educate, advocate, and guide our patients. 🫶🏽

Here’s a breakdown I always keep in mind when working with insulin:

✨ Rapid-Acting:

• Kicks in FAST (10-30 min)

• Examples: Humalog, Novolog, Lyumjev

• Usually taken with meals

✨ Short-Acting:

• Starts in 30-60 min

• Example: Regular insulin

• Sometimes called “mealtime insulin” too

✨ Intermediate-Acting:

• Takes 1-2 hrs to start

• Example: NPH (less common now)

• Usually cloudy — roll it before use!

✨ Long-Acting:

• Covers all day (basal)

• Examples: Lantus, Toujeo, Levemir, Basaglar

• Taken once daily

✨ Ultra-Long Acting:

• Lasts even longer (up to 42 hrs)

• Example: Tresiba

There’s so much power in knowing what these meds do — not just for us as techs, but for the patients who depend on them every day. 🧠💪🏼

... Read moreBeyond just knowing the types of insulin, I've noticed a huge difference when patients truly understand how to use their insulin, especially with all the different pen devices out there! From my experience working in the pharmacy, insulin pens have revolutionized diabetes management for so many people. They're incredibly popular for their ease of use and convenience. Whether it's a pre-filled pen you dispose of after it's empty or a reusable pen where you replace the cartridge, they offer accurate dosing with a simple click. I've personally seen how much easier these pens make managing diabetes for many people. No more drawing from vials for some, just click and inject! It's especially true for those rapid-acting ones that need to be taken right before meals. My patients often ask about specific rapid-acting pens. For instance, the Humalog KwikPen (insulin lispro) and Novolog FlexPen (insulin aspart) are super common. They’re designed to be taken right before or even shortly after meals to help cover the carb intake quickly. But then there's Fiasp FlexTouch, which I've seen is even faster than traditional rapid-acting insulins – sometimes called ultra-rapid acting. It's truly amazing how quickly it gets to work, often within minutes, which can offer more flexibility around meal times. Other rapid-acting insulins like Admelog and Apidra (insulin glulisine) also come in pen forms, providing similar quick action. Knowing these nuances really helps patients plan their day and manage post-meal blood sugar spikes effectively. And for those basal needs, pens like the Lantus SoloStar (insulin glargine) and Tresiba FlexTouch (insulin degludec) are game-changers. I often explain to patients that these are like their background insulin, keeping blood sugar steady all day or even longer with Tresiba, which can last up to 42 hours! It's so important to take these consistently, usually once a day, to provide that essential baseline coverage. Something that frequently comes up, especially with older regimens or in specific cases, is the mixing of NPH and Regular insulin. While less common now with newer analog insulins, it's still a technique some patients use. From my experience, the biggest takeaway here is the sequence: 'clear before cloudy.' This means drawing the clear (Regular) insulin first, then the cloudy (NPH) insulin. Why? To avoid contaminating the Regular insulin vial with NPH. It's a precise process, and using the correct insulin syringe is absolutely vital for accurate dosing. I always emphasize that patients should never attempt this without thorough training from their doctor or an educator, as incorrect mixing can lead to serious blood sugar fluctuations. It’s not just about knowing the steps; it's about understanding the 'why' behind each one. No matter what type of insulin pen you're using, proper storage is key. Unopened pens belong in the fridge. Once opened, most are good at room temperature for about 28 days, but always check the specific package insert for your brand! And don't forget to rotate injection sites to prevent lipohypertrophy. These small details can make a huge difference in how effective your insulin is. Ultimately, my biggest advice is to always ask questions. Your pharmacy tech, nurse, or doctor is there to help you understand every aspect of your insulin therapy. Knowing your rapid-acting from your long-acting, how your pen works, and proper mixing techniques truly empowers you to manage your diabetes effectively. It's a journey of continuous learning, and we're here to guide you.
