Why We Count ( & Count Again) as a nurse in the OR

One of the most important things we do in the OR is count.

Sponges, sharps, instruments—everything has to be accounted for. Not once, but multiple times throughout the case. It might seem tedious, but it’s one of the biggest ways we keep our patients safe.

As the circulator, I’m always double-checking with my scrub tech and making sure nothing gets left behind. Even in chaotic or rushed moments, we slow down and stick to the process. Because at the end of the day, it’s about making sure our patients leave the OR better than they came in.

This job is all about the details—and this is one of those small things that makes a huge difference. #lemon8partner #OperatingRoomNurse #SurgicalCount #PatientSafetyAlways #NurseStrong #ScrubAndCirculate

2025/4/24 Edited to

... Read moreIn the operating room (OR), meticulous counting of instruments, sponges, and sharps is non-negotiable for ensuring patient safety. Surgical counts are conducted before, during, and after procedures, significantly reducing the risk of retained surgical items (RSIs). RSIs can lead to serious complications, including infections and additional surgeries for removal. An effective count procedure involves both the circulating nurse and the scrub tech, who must communicate clearly and consistently. This practice fosters teamwork and reinforces accountability, ensuring that every item used in the procedure is accounted for. In chaotic surgical environments, the commitment to precise counting can seem tedious; however, it is vital for maintaining focus and a systematic approach to patient care. Furthermore, continuous education and drills on counting protocols can help reinforce the importance of these practices among surgical team members. Institutions often implement regular training sessions to keep staff updated on the latest best practices for patient safety and surgical counts. Enhanced awareness and understanding of the count process not only improve safety outcomes but also empower nurses and surgical teams, cultivating a culture of safety within the operating room. Ultimately, effective surgical counting contributes to higher efficiency and better patient care, as it minimizes risks and fosters a culture where patient well-being is the top priority. Always remember: in the OR, every count matters.
