ICD is the same but not the same!!!
ICD is not the same! Don't confuse between "heart jerk" and "thoracic drain line."
Clinically, the term ICD has two meanings.
If not clearly identified, discrepancies can affect patient safety.
1️⃣ ICD Heart Electric Jerk Machine
(Implantable Cardioverter Defibrillator)
🫀 What is?
The subcutaneous implantation device serves 24 hours of heart surveillance.
If the ventricular fibrillation →, the machine will shock or stimulate the electricity to return to normal rhythm immediately.
🎯 used in who?
• Ever cardiac arrest / VF / VT
• Low EF, heart muscle weakness
• Genetic disease risks cardiac arrhythmia
🚨. Very important.
ICD "not used" treatment AF (atrial arrhythmia)
⚠️ Nursing to know.
• If the machine has Beep sound → Hurry and send the device clinic
• Swollen wound, red, pus → Do not sheep themselves
• The phone uses the opposite ear.
• The airport must be notified to the authorities.
🧠 Heart:
The ICD is to "prevent acute death."
Because the brain can withstand hypoxia for a few minutes.
2️⃣ ICD thoracic drain line
(Intercostal Drainage / Chest Tube)

Emergency operations. Put a line to drain.
Wind / blood / water / pus out of chest cavity
To get the lungs expanded back.

• Pneumothorax (especially Tension must be done immediately)
• Hemothorax
• Empyema
• Pleural effusion, large volume

💧 Water Seal System to remember
• The end of the tube is 2 cm deep.
• Must see water ripple by breathing (Oscillation)
🚨 key 1.5-liter rule.
• Do not drain more than 1.5 L the first time.
• Not faster than 0.5 L / h
To prevent the Re-expansion Pulmonary Edema.
❌ Absolutely forbidden.
• Do not push back the strings
• Do not wear new lines, original holes
• Do not Clamp if there is a leakage

เสริมจากประสบการณ์ในวอร์ด: เวลามีคนพูดว่า “คนไข้ on ICD” หรือ “ทำ ICD แล้ว” ให้รีบถามต่อทันทีว่า ICD ไหน—เครื่อง ICD (หัวใจ) หรือสาย ICD/Chest tube (ทรวงอก) เพราะสองอย่างนี้คนละเรื่องและความเสี่ยงคนละแบบมาก ๆ ถ้าสื่อสารไม่ชัดอาจกระทบ patient safety ได้เลย 1) “on ICD” คืออะไรกันแน่? - ถ้าหมายถึง “ผู้ป่วยใส่เครื่อง ICD หัวใจ” มักเป็นเคสที่ฝังอุปกรณ์ใต้ผิวหนัง (แถวไหปลาร้าหรือรักแร้) เพื่อเฝ้าระวังและช็อกเมื่อเกิด VT/VF (ไม่ได้ใช้รักษา AF) จุดที่ต้องเน้นคือซักอาการช็อก (ช็อกกี่ครั้ง/ถี่ไหม/มีวูบไหม), ดูแผล บวมแดงร้อนเจ็บ/มีหนอง, ฟังคำว่าเครื่องร้อง beep แล้วอย่ารอ—ส่งคลินิกอุปกรณ์หรือให้แพทย์ประเมินทันที - แต่ถ้าเป็น “on ICD (Intercostal Drainage)” แปลว่ามีสายระบายทรวงอกอยู่ ต้องโฟกัสเรื่องระบบขวด/น้ำ water seal/ลมรั่ว/ปริมาณที่ระบายและอาการหอบเหนื่อย 2) การต่อ ICD/Chest tube แบบต่าง ๆ (ทำไมมี 1 ขวด vs 2 ขวด) โดยหลักในวอร์ดจะเจอ “ระบบปิด (closed drainage)” ที่แบ่งเป็นส่วนสำคัญ ๆ คือ - Collection chamber: ห้องรองรับของเหลวที่ระบายออกมา - Water seal: ทำหน้าที่เป็น “วาล์วน้ำ” กันลม/ของเหลวย้อนกลับ และช่วยบอกการทำงานของระบบ - Suction control (บางเคส): ใช้แรงดูดช่วยให้ปอดขยายตัว (ต่อกับ wall suction ตามแผนแพทย์) แบบ 1 ขวด (Single bottle) - หลักคิด: ขวดเดียวทำทั้งเก็บของเหลวและ water seal - ข้อควรระวัง: ถ้าระบายของเหลวเยอะ ระดับน้ำในขวดสูงขึ้น อาจเพิ่มแรงต้าน ทำให้ระบายได้ไม่ดีเท่าที่ควร (เลยมักใช้ในเคสที่คาดว่าของเหลวไม่มาก) แบบ 2 ขวด (Two bottle) ที่หลายคนค้นหา “การต่อ ICD 2 ขวด” - หลักคิด: แยก “ขวดเก็บของเหลว” ออกจาก “ขวด water seal” - ข้อดี: ต่อให้ของเหลวออกเยอะ ก็ไม่ทำให้ระดับน้ำใน water seal เพี้ยน ระบบเสถียรกว่า เหมาะกับเคสมีน้ำ/เลือดออกต่อเนื่อง - จุดที่ต้องเช็กให้ชัวร์ตอนรับเวร: สายจากคนไข้ไปขวดเก็บของเหลวก่อน แล้วค่อยต่อไปขวด water seal (ทิศทางถูกต้อง), ขวดตั้งต่ำกว่าระดับหน้าอกเสมอ และสายไม่พับงอ/ไม่ถูกกดทับ หมายเหตุ: ปัจจุบันหลายโรงพยาบาลใช้ “ชุดขวดสำเร็จรูป 3 ช่อง” ในกล่องเดียว ซึ่งแนวคิดเทียบได้กับ 2–3 ขวดแบบเดิม ให้ดูคู่มือของอุปกรณ์ที่หน่วยงานใช้เป็นหลัก 3) เช็กลิสต์เร็ว ๆ สำหรับการพยาบาลผู้ป่วย on ICD (Chest tube) - Keep it low & closed: วางขวดต่ำกว่าหน้าอก ห้ามเปิดระบบโดยไม่จำเป็น - ดู oscillation/fluctuation: ระดับน้ำใน water seal กระเพื่อมตามหายใจได้ (ถ้าไม่กระเพื่อม อาจสายตัน/ปอดขยายเต็ม/หรือระบบมีปัญหา) - ดู air leak: ฟองอากาศใน water seal ถ้ามี “ต่อเนื่อง” ให้สงสัยลมรั่ว (ดูจุดต่อทุกจุดก่อน แล้วค่อยรายงาน) - วัดและบันทึก: สี/ลักษณะ/ปริมาณน้ำที่ออกเป็นช่วงเวลา สังเกตเลือดสดออกมากผิดปกติให้รีบแจ้งแพทย์ - ห้าม clamp มั่ว ๆ: โดยเฉพาะถ้ายังสงสัยลมรั่ว เพราะเสี่ยง tension pneumothorax - ถ้าสายหลุด: ห้ามดันกลับเข้าไปเอง ให้ทำแผลปิดแบบป้องกันอากาศเข้าตามแนวทางหน่วยงานและรีบเรียกทีมแพทย์ 4) คำที่ช่วยลดความสับสนเวลาสื่อสารในทีม เวลารายงาน/โทรเรียก ควรพูดให้ครบแบบนี้เสมอ - “เครื่อง ICD หัวใจ (Implantable cardioverter defibrillator)” หรือ - “สาย ICD/Chest tube (Intercostal drainage) ต่อระบบ…(1 ขวด/2 ขวด/ชุดสำเร็จรูป) มี/ไม่มี suction” สรุป: ICD เหมือนกันแค่ตัวย่อ แต่ความหมายต่างกันสุด ๆ ถ้าทำให้ชัดตั้งแต่คำแรก ทีมทำงานง่ายขึ้นและคนไข้ปลอดภัยขึ้นจริง ๆ
