Hemorrhagic strokes result from bleeding caused by a ruptured blood vessel within or around the brain. Recognizing stroke type is vital for timely treatment and preventing complications. Nursing care priorities focus on airway management, neurological assessments, monitoring vital signs, and ensuring patient safety. Frequent neuro checks using tools like the NIH Stroke Scale help detect any changes early. Continuous monitoring of oxygen saturation, respiratory rate, and lung sounds supports respiratory function, while blood pressure management is critical—especially with ischemic strokes where "permissive hypertension" may be allowed to maintain cerebral perfusion. Avoiding rapid BP reductions is emphasized to prevent worsening ischemic injury. Specific nursing interventions address complications such as aspiration risk, which is heightened by swallowing difficulties common in stroke patients. Elevating the head of the bed to 30-45 degrees and maintaining suction equipment at bedside are standard care practices to reduce the risk of aspiration pneumonia. Additionally, glucose control within target levels of 140-180 mg/dL is necessary since imbalances can worsen neurologic outcomes. The transient ischemic attack (TIA), or mini-stroke, involves temporary blockage with stroke-like symptoms that resolve within 24 hours. It serves as an important warning sign requiring risk-factor management such as hypertension, diabetes, and lifestyle modification to prevent full strokes. Overall, stroke nursing care integrates rapid assessment, safety measures like fall prevention and skin care, infection monitoring, and clear communication strategies using simple commands. Educating patients and families on stroke warning signs using BE FAST—balance, eyes, face drooping, arm weakness, speech difficulties, and time to call emergency services—can empower early recognition and immediate intervention, crucial for improving recovery outcomes.
