When we think about stroke, we often think about speech or movement — but swallowing is one of the most commonly affected functions.

Research shows that over half of people experience dysphagia immediately after a stroke, and for some, swallowing problems can persist long-term.

Why this matters 👇

These symptoms may indicate silent aspiration, where harmful material enters the airway without triggering a cough reflex, significantly increasing the risk of aspiration pneumonia — a serious lung infection. Early assessment by a speech-language pathologist is essential. Dysphagia screening within the first 24 to 48 hours post-stroke helps identify those at risk. Instrumental evaluations like the Videofluoroscopic Swallow Study (VFSS) or Fiberoptic Endoscopic Evaluation of Swallowing (FEES) further detail the specific physiologic impairments, such as delayed pharyngeal swallow triggers, reduced hyolaryngeal elevation, or poor pharyngeal constriction. These findings guide targeted therapy. Compensatory strategies, like upright positioning during meals or chin tuck techniques, often protect the airway initially. However, restorative exercises tailored to deficits—for example, Mendelsohn Maneuvers for upper esophageal sphincter dysfunction or effortful swallows to strengthen throat muscles—can improve swallowing safety and efficiency over time. Importantly, recovery timelines vary. While many improve within weeks, a considerable subset experiences persistent dysphagia months after stroke, necessitating ongoing evaluation and care. Caregivers also play a critical role in monitoring subtle signs and advocating for reassessments every 4 to 8 weeks. Understanding the complexity of swallowing disorders post-stroke and involving specialized speech therapy provides a pathway to safer eating, better nutrition, and improved quality of life. If you or a loved one is recovering from stroke, pay attention to swallowing signs and seek expert support promptly.
