悪性リンパ腫の看護計画は多岐にわたり、実際に現場で戸惑う学生も多いですが、3つの視点を意識するだけで大きく理解が進みます。 まず、症状から計画を立てる際は、患者さんの腫瘤や臓器浸潤による痛み、発熱・高体温などのB症状、貧血による疲労感や活動耐性の低下などを丁寧に評価しましょう。実習中、これらの症状を具体的にイメージし、どの看護問題に結びつくかを考えることが重要です。 次に、治療面からはR-CHOP療法などの抗がん剤治療の副作用予測を行い、骨髄抑制による感染や出血リスク、口腔粘膜炎や皮膚障害、悪心・嘔吐、脱毛、食欲不振などの潜在的な問題に先手を打ったケアを計画します。私も実習でケアプランを考える際、事前にこれらを 整理しておくことで、指導者からの質問にも自信を持って答えられました。 そして心のケアも欠かせません。悪性リンパ腫の診断や治療による不安、役割葛藤、治療法選択の悩み、社会的役割の変化による喪失感など、多様な心理・社会的側面を観察し、患者さんの思いに寄り添う支援が必要です。 これら3つの視点を組み合わせて全体像を把握することで、悪性リンパ腫患者の看護計画がスムーズに立てられ、実習での経験も確実に力になります。ぜひこの記事を保存し、実習の際に何度も見返して、自信を持って看護計画に取り組んでくださいね。
