脳出血の患者さんに対する非効果的健康管理の問題は、単に知識を伝えるだけでなく、患者の個別の状況や環境を踏まえた丁寧なアセスメントとケア計画の立案が重要だと感じています。 たとえば小脳出血の場合、症状の特殊性から身体機能の低下だけでなく精神的な不安や社会資源の活用状況まで幅広く観察する必要があります。私の実習では、患者のADLの現状や生活環境を綿密に把握し、その上で具体的な生活習慣改善や服薬管理の指導を行いました。 特に教育計画（EP）には時間をかけ、患者さん本人とそのご家族が再発予防の重要性を理解し、日常生活で自立的に管理できるよう支援することが欠かせません。社会資源の案内も積極的に取り入れて、患者の安心感を高められた経験があります。 このような多面的アプローチを通じて、患者さんの「できる」を増やしていくことこそが、非効果的健康管 理を克服し、再発予防につながると思います。皆さんの看護計画作成の参考になれば幸いです。
