2. Advanced care directive - includes wishes of the living person with illness. Do they want CPR/life saving precautions or do they want to focus on comfort with death?

3. Authorized representative (for Health Care Authorities & private or state insurance) - allows you to access information/speak selectively on their behalf.

One document that became a cornerstone for me was the DSHS Authorized Representative form. If you're like me, you've probably searched for help with this exact form, wondering about its purpose and how to complete it correctly. Understanding the Health Care Authority Authorized Representative is key. This isn't just another piece of paper; it's your official permission slip to act on behalf of your loved one when dealing with the Department of Social and Health Services (DSHS) or the Health Care Authority (HCA). In my experience, this form was invaluable for tasks like discussing eligibility for various programs, receiving renewal forms, and ensuring continuity of care. The form itself, often identified by the DSHS 14-532 code, requires specific information. You'll need your loved one's client information, including their full name and ACES CLIENT ID NUMBER. Then, you'll fill in your details as the authorized representative: your name, organization (if applicable), phone number, and mailing address. This ensures DSHS knows exactly who they can speak with regarding your loved one's case. Next, you need to designate which programs you want to represent them for. This is crucial and can include Cash Benefits, Basic Food Benefits, Health Care Coverage, or Long-term Care Coverage. Don't skip this part! I learned the hard way that if you don't check the right boxes, you might not be able to access the information you need for a specific service. You also decide the duration of your authorization – it can be for a specific period like 90 days or remain ongoing. For ongoing representatives, remember that cash and basic food authorizations expire annually, while medical coverage continues until revoked or upon the individual's passing. One thing I found particularly helpful to understand from the details was the section on HIPAA restrictions. Even with an Authorized Representative form, there are limits. For instance, mental health information, STD test results, or chemical dependency services usually require additional specific consent. This is important to know so you don't get frustrated if certain information isn't immediately shared. Empowering yourself with this document means you can receive DSHS/HCA letters, discuss eligibility, and even manage ProviderOne cards. It helps streamline communication and prevents delays in accessing crucial benefits and services. I also found solace in knowing I could remove or cancel my request for authorization at any time, giving both my loved one and me peace of mind. Filling out forms like the DSHS Authorized Representative isn't anyone's favorite task, but it’s a necessary step in effective caregiving.
