Understanding the nuances between peritoneal dialysis (PD) and hemodialysis (HD) was one of my biggest challenges during nursing school, especially when preparing for the NCLEX! It’s not just about what they are, but why a patient might undergo one over the other. When we talk about 'why peritoneal dialysis instead of hemodialysis,' it often comes down to patient-specific factors and lifestyle. PD, for instance, uses the body's own peritoneal membrane as a natural filter. It's often preferred for patients who might not tolerate the rapid fluid shifts that are common with hemodialysis. Imagine a patient with significant cardiovascular issues; the sudden removal of large volumes of fluid during HD could be too stressful for their heart. PD offers a gentler, more continuous filtering process, which can be done at home, giving patients more independence and flexibility. I've heard stories from patients who appreciate not having to travel to a clinic multiple times a week. On the other hand, hemodialysis is usually performed in a clinic setting, typically 3-4 times per week. It's incredibly efficient in cleansing the blood, but as mentioned in our cheat sheet, it requires patients to be anticoagulated and involves significant fluid shifts. A crucial aspect for nurses is managing the patient's vascular access – usually a fistula or graft. We learn to monitor for complications, palpate a thrill, and auscultate a bruit to ensure the fistula is patent and functioning. This is something you definitely won't be doing with PD patients! Deciding between PD and HD involves assessing a patient's overall health, cognitive ability (for managing home PD), living situation, and personal preferences. While PD offers flexibility, it comes with a higher risk of peritonitis if sterile techniques aren't followed meticulously when handling the dialysate. HD offers closer medical supervision during treatment but requires a more rigid schedule and can impact a patient's energy levels after sessions. As future nurses, understanding these 'whys' helps us provide holistic care and educate our patients effectively, guiding them through what can be a daunting treatment decision.
