One major factor contributing to this challenge is masking, where women learn to hide their symptoms by developing perfectionistic tendencies, overachieving academically or professionally, and managing emotional responses to appear socially compliant. This often leads to chronic stress and burnout as women juggle multiple responsibilities while struggling internally. Hormonal fluctuations play a significant role in modifying ADHD symptoms for many women. Changes in estrogen levels impact dopamine regulation, crucial for attention and emotional control, causing symptoms to intensify during menstrual cycles, postpartum periods, or perimenopause. This hormonal effect can result in inconsistencies in focus, memory, and mood stability, which might be misinterpreted as mood disorders rather than manifestations of ADHD. Another distinctive feature is the predominance of inattentive symptoms such as brain fog, daydreaming, and disorganization, which contrast with the external hyperactivity more commonly observed in boys. Women with ADHD may appear quiet or disengaged but internally experience racing thoughts and mental fatigue. Rejection sensitivity dysphoria (RSD) also disproportionately affects women with ADHD. This emotional hypersensitivity can lead to intense feelings of self-doubt, emotional outbursts, or withdrawal following perceived criticism or rejection. These reactions are often mistaken for anxiety or mood disorders, complicating accurate diagnosis and treatment. Overall, recognizing these five key differences—inattentiveness, masking, hormonal impact, over-functioning, and rejection sensitivity—is essential for better support and treatment of women with ADHD. Awareness of these nuanced presentations can lead to earlier diagnosis, personalized approaches to management, and improved mental health outcomes.
