🧬 When tapeworms become cancerous - and what it quietly tells us about AML.

A landmark article in the New England Journal of Medicine (2015) documented a medical case that had never been found in history.

A 41-year-old man in Colombia who is infected with HIV causes necrosis nodules throughout the body - but those cancer cells are not human cells; they are of Hymenolepis nana origin or "dwarf tapeworms," marking the world's first case in which a human fell ill from cancer cells that grew and mutated from parasites in the body.

Here's what science proves from this case:

Deep Sequencing found a parasitic gene mutation that accurately mimics human cancer behavior, where parasitic stem cells invade human tissue, divide like crazy, and lose control, turning themselves into cancer cells.

So how does this have to do with AML leukemia?

The answer is that there is an amazing indirect connection if we look at it through biological perspective:

Both this tapeworm case and AML have an interesting "evolutionary parallel structure." When a stem cell is faced with extreme environmental pressures, it begins to accumulate mutations that lose control and begin to expand without limits.

▪️ in the corner of AML: caused by embryonic stem cells in the blood system accumulate mutations. Also known as Clonal Hematopoiesis (CH), which is like the origin of "pre-cancer cells" ready to erupt into full leukemia when given added stimulating factors.

▪️ in the corner of the tapeworm: Being in the body of a severely immunocompromised host becomes a catalyst for the worm's stem cells to mutate and divide unregulated to eventually become necrosis.

Both stories run to converge on the same truth:

Immunodeficiency is not just the weakness of the body, it is the "fine environment" that selects and allows mutant cells to grow, whether they come from the human bone marrow or from parasites hidden within us.

3 key questions this case leaves to AML research:

1. Are we underestimating the role of "infectious disease" as a co-driver of exacerbated disease symptoms? Currently, we know that 30% of AML patients have preceding Clonal Hematopoiesis, but chronic infection or parasite burden is an environmental factor that has been studied very rarely as an accelerator for cancer.

2 Is there a DNA of other organisms hidden in Idiopathic AML patients? In remote, resource-poor areas with immunodeficiencies and parasites, is it possible that some patients have AML-like symptoms, but they are actually caused by parasites, and may be cured in a different way?

3 In oncology, where does "Self" end? This case completely negates the line between the host (human) and the foreign pathogen, because it is the real cancer cells that hurt humans, but there is a genetic blueprint from the parasite, which our current diagnostic system is not designed to deal with.

Of course, this is just a single case report, which does not necessarily mean that helminthosis directly causes leukemia, but it opens the door for us to study how the immune environment (Immune Microenvironment) affects the evolution of leukemia.

Because sometimes...The rarest cases often give us the most universal lessons.
