When there is a crash, a jam, a delay, or a specific problem, the most important thing may not be to ask:

"How did that happen?"

"Who did it?"

"Why miss?"

"Why not prepare better?"

Because these questions, while important, may not be the ones to ask first.

Like a fire burning a house, the first thing to do is to extinguish the fire, prevent it from spreading, and save people from dangerous areas, not to discuss which plug the fire started, who plugged it, or who should be responsible.

So when there are problems in the job, let's start with the question:

"What can we do now to improve the situation and continue the work?"

Solve what's solved first.

Manage the impact that is happening.

Find a backup option

Help each other take work through the jam point.

And then when the situation calms down, come back and review what happened, why, where systems are vulnerable, and how to prevent them from recurring.

Not rushing to find the wrong person doesn't mean not being responsible.

And solving a particular problem doesn't mean you don't have to find a cause.

Just need to know what should come first and what should come later.
