Have you ever wondered, when we get cut by a knife or fall and scrape, the blood can stop flowing! That's because our bodies have a "Super Hero" system called Hemostasis. 🦸♂️💥
Let's see how our bodies work. It's easy to divide into two steps.
🔴 Step 1: Platelet Army (Primary Hemostasis)
It's like putting a piece of tape on a temporary wound.
1. Contracted blood vessels: Once you know the injury, the blood vessels will immediately constrict to slow the blood flow. 📉
2. Platelet pools: The platelets run to the wound and then "stick" together to become a cork (Platelet Plug) to plug the leak.
3. The blood will start to seep away. 🩹
🔴 Step 2: Secondary Hemostasis
When the temporary adhesive tape may not be tight enough, the body will create an "elephant seal glue" to add! 1.Coagulation Cascade: It is a forward-looking function of blood protein (Clotting Factors).
2. Create Fibrin: The body creates a fiber called "Fibrin," which looks like a sticky net, and covers the platelets again.
3. Blood clot clots: This net traps blood cells until they become a strong "blood clot." The wound closes completely and begins to heal. 💪

ถ้า “เลือดไหลไม่หยุด” หลายคนจะตกใจมาก (เราเองก็เคย) เลยลองสรุปแบบใช้งานได้จริงว่าเกิดจากอะไรได้บ้าง + ควรทำยังไงก่อนถึงมือหมอ โดยโยงกับกลไกห้ามเลือด 2 ขั้น (Primary/Secondary Hemostasis) ที่เล่าไว้ในโพสต์นี้ 1) สาเหตุที่ทำให้เลือดหยุดช้าหรือไหลไม่หยุด (พบได้บ่อย) - กดแผลไม่ถูกวิธี/กดไม่ต่อเนื่อง: ช่วงแรกเกล็ดเลือดกำลังรวมตัวเป็น “จุก” (Platelet plug) ถ้าเรายกผ้าดูแผลบ่อยๆ จุกที่กำลังก่อตัวจะหลุด เลือดก็ไหลต่อ - แผลลึก/กว้าง หรือโดนเส้นเลือด: ต่อให้มี platelet plug แต่รอยฉีกใหญ่เกินไป เลือดจะชนะการอุดชั่วคราว - ยาต้านเกล็ดเลือด/ยาละลายลิ่มเลือด: เช่น aspirin, clopidogrel หรือยากลุ่ม warfarin/DOACs ทำให้ขั้น Primary หรือ Secondary Hemostasis ทำงานช้าลง (ตาข่ายไฟบรินสร้างไม่ดี) - เกล็ดเลือดต่ำ/การทำงานของเกล็ดเลือดผิดปกติ: ทำให้ “Step 1” ไม่แน่น เลือดซึมง่าย ฟกช้ำง่าย - ขาดวิตามิน K หรือโรคตับ: วิตามิน K สำคัญต่อการสร้าง clotting factors หลายตัว ถ้าขาดหรือสร้างไม่ได้ จะกระทบ “Coagulation cascade” และการสร้างไฟบริน 2) วิธีปฐมพยาบาลเมื่อเลือดไหลไม่หยุด (ทำตามนี้ก่อน) - กดตรงแผลด้วยผ้าสะอาด/ผ้าก๊อซ “แรงพอและต่อเนื่อง” 10 นาทีเต็ม อย่าเพิ่งยกดู - ถ้าเลือดซึมทะลุผ้า ให้ “เพิ่มผ้าซ้อน” แล้วกดต่อ ไม่ต้องดึงผ้าชั้นแรกออก - ยกบริเวณที่เป็นแผลให้สูงกว่าระดับหัวใจถ้าทำได้ เพื่อลดแรงดันเลือด - ถ้าเป็นเลือดกำเดา: นั่งเอนไปข้างหน้า บีบปีกจมูกส่วนอ่อน 10 นาทีเต็ม (ไม่เงยหน้า) 3) สัญญาณอันตรายที่ควรรีบไปโรงพยาบาล - กดแน่นต่อเนื่อง 10–15 นาทีแล้วยังไม่หยุด หรือเลือดพุ่งเป็นจังหวะ - แผลลึก เห็นชั้นไขมัน/กล้ามเนื้อ ขอบแผลอ้า หรือสงสัยโดนเส้นเลือด - มีอาการหน้ามืด ซีด เหงื่อแตก ใจสั่น หรือสงสัยเสียเลือดมาก - มีจ้ำเลือดขึ้นง่ายผิดปกติ เลือดออกตามไรฟันบ่อย ประจำเดือนมากผิดปกติ หรือมีประวัติเลือดออกง่ายในครอบครัว - กำลังกินยาละลายลิ่มเลือด/ยาต้านเกล็ดเลือด หรือมีโรคตับ ไต 4) เกี่ยวอะไรกับ PT/aPTT? ถ้าหมอสงสัยว่าปัญหาอยู่ที่ “Secondary Hemostasis” (coagulation cascade/การสร้างไฟบริน) มักตรวจ PT/INR และ aPTT เพื่อดูว่าปัจจัยการแข็งตัวของเลือดทำงานปกติไหม โดยเฉพาะในคนที่ใช้ warfarin ขาดวิตามิน K หรือมีโรคตับ สรุปสั้นๆ: เลือดไหลไม่หยุดอาจเกิดจากกดแผลไม่ถูก แผลใหญ่ หรือระบบห้ามเลือด (เกล็ดเลือด-ปัจจัยการแข็งตัว-ไฟบริน) มีปัญหา ถ้าเข้าข่ายสัญญาณอันตราย อย่ารอดูอาการนาน รีบไปพบแพทย์จะปลอดภัยที่สุด
