Can Emotional Pain Become Physical Pain?

Emotional pain can be defined as being aware that one is experiencing negative emotions such as fear, despair, grief, shame, guilt, blocked love, loneliness and loss. Emotional pain can occur alongside mental disorders such as depression which affects a person's emotional state.

Physical pain is defined by an “unpleasant feeling in our body that makes us want to stop and change our behavior”. It may result from acute tissue injury, persistent inflammation, or nervous system dysfunction. One of the most common ways to measure pain in Singapore is through the Numeric Rating Scale where a score of 0 indicates "no pain," while 10 signifies the "worst pain imaginable” This shows that pain is often subjective.

It is important to note that the International Association for the Study of Pain recognises pain as both a sensory and emotional experience.

Based on these definitions alone, emotional pain and physical pain can coexist and one may present itself as the other. However the word ‘become’ in the question suggests a transformation or change from emotional pain to physical pain. So how does emotional pain become physical pain?

An example of emotional pain that many of us may have experienced is deep grief caused by the loss of a loved one, pet, colleague or close friend.

When experiencing grief, a person may feel sadness, numbness, shock, anger or despair. Losing someone hurts and it can feel unfair. The overwhelming thoughts and emotions related to grief can place a lot of stress on the mind and body.

At a certain point, the emotional burden may become so overwhelming that the body begins to react physically. These reactions can include crying, oversleeping, insomnia, hitting objects, or social withdrawal.

Crying: This can lead to headaches due to the release of stress hormones and increased muscle tension around the head and face.

Excessive Sleeping: Lying in bed for prolonged periods may cause stiffness due to reduced blood flow and inactivity. Oversleeping may also contribute to tension headaches.

Lack of Sleep: Insomnia is associated with headaches, fatigue and eye- related issues like dye-eye syndrome where the eyes experience “redness, irritation, and a gritty sensation.”

Hitting Objects: This may be a form of self -harm where a person intentionally inflict pain or injury on themselves without suicidal intent. Such actions may cause bruises, fractures or other physical injuries.

Social withdrawal: This is a response to the stress and trauma of the loss. If prolonged, there can be long term health consequences. Studies have shown that stress and isolation can increase the risk of heart disease, stroke, type 2 diabetes, and other illnesses that may present with physical pain and discomfort.

Ultimately, it is widely understood that emotional pain can manifest itself as physical pain due to the stress response and hormones that the body releases during this time. Emotional pain may also become physical through the ways individuals respond to their emotions.

As a result emotional suffering can become bodily pain, whether directly through stress-related physiological changes or indirectly through harmful coping mechanisms.

We can see that emotional pain and physical pain are deeply interconnected.

In fact, a study by Harvard University found that the first 24 hours after bereavement are associated with a staggering 21-fold increase in the risk of a heart attack.

Insomnia brought on fatigue and headaches, while oversleeping caused stiffness and discomfort due to inactivity. It shows how emotional struggles often cascade into physiological symptoms. Another way emotional pain converts to physical pain is through coping mechanisms. For some, it might be withdrawing socially, which research shows raises risks of conditions linked with physical pain, like heart disease and diabetes. For others, it might involve self-harm or hitting objects as an outlet for anguish, resulting in bruises or injuries. These behaviors underline the complex pathways from emotional distress to body pain. What has helped me personally is recognizing these connections early and adopting techniques aimed at reducing stress responses, such as mindfulness, gentle exercise, and seeking social support. Understanding that emotional and physical pain are deeply intertwined encourages a holistic approach to healing. It’s crucial to acknowledge and address emotional wounds as part of physical health care, ensuring the mind and body are treated as one integrated system.
