Neurology horror edition
Cases that still run through my mind weekly 💔
1. Gaze deviation to the right + left-sided plegia
→ large right hemispheric stroke
2. Awake, tracking with eyes, but unable to move or speak, communicates only with blinking
→ locked-in syndrome (brainstem stroke)
3. Patient presents with sudden unilateral ptosis + dilated pupil + “down and out” eye
→ CN III palsy → think aneurysm until proven otherwise
4. Rapidly progressive dementia over weeks–months, myoclonus, ataxia, bizarre behavior
→ Creutzfeldt-Jakob disease (CJD)
5. Progressive weakness that starts focally (hand clumsiness, foot drop), now spreading, with muscle atrophy + fasciculations but preserved sensation
→ Amyotrophic Lateral Sclerosis
