Liver cancer, pancreatic cancer and cholangiocarcinoma
Hepatocellular cancer, pancreatic cancer and cholangiocarcinoma are virulent cancers with a high mortality rate in Thailand, each with different characteristics and causes as follows:
1.Hepatocellular carcinoma, HCC
- caused by abnormal liver cells, rapidly growing and spreading cancer cells.
- The liver performs important functions such as eliminating toxins, creating proteins, and helping digestion.
- Major causes such as cirrhosis, hepatitis B and C infection, heavy alcohol consumption, including carcinogens in food.
- It usually occurs without symptoms at an early stage and is usually seen when the disease has spread, such as upper right abdominal pain, weight loss, anorexia, nausea.
2. Cholangiocarcinoma
- caused by bile duct lining cells that convey bile from the liver to digest fat in the small intestine.
- The main cause in Thailand is liver flukes infection from the consumption of raw fish or unclean raw collards.
- There are two types: cholangiocarcinoma inside the liver and cholangiocarcinoma outside the liver.
- Early symptoms are not obvious, but when the lump grows, there will be yellowing, yellow eyes, itching, pale stools, weight loss.
3. Pancreatic cancer
- Mostly the type caused by pancreatic duct cells (Ductal Adenocarcinoma).
- The pancreas is near the bile duct. When a cancer in the head of the pancreas presses on the bile duct, it causes lymphatics, bile, oily feces.
- Symptoms of early pancreatic cancer are often unclear, with abdominal pain, fatigue, weight loss, metastatic spread to the liver and other organs.
- Diagnosis using CT scans, MRI, ultrasound and biopsy.
In general, these cancers often show no early symptoms, making them slow to detect, resulting in difficult treatment and high mortality rates; the key prevention is to avoid risk factors such as hepatitis infection, eating raw fish with liver flukes, as well as regular health and disease screening for early diagnosis and treatment.
มะเร็งตับ มะเร็งตับอ่อน และมะเร็งท่อน้ำดีจัดเป็นกลุ่มโรคมะเร็งที่มีอัตราการเสียชีวิตสูงและพบได้บ่อยในประเทศไทย โดยเฉพาะมะเร็งท่อน้ำดีที่สัมพันธ์โดยตรงกับปัจจัยเสี่ยงเฉพาะถิ่น เช่น การติดเชื้อพยาธิใบไม้ในตับจากการบริโภคปลาดิบที่ไม่สะอาด ในทางการแพทย์ การวินิจฉัยโรคเหล่านี้มักทำได้ด้วยเครื่องมือทันสมัยอย่าง CT scan, MRI และอัลตราซาวนด์ ซึ่งช่วยตรวจหาก้อนมะเร็งหรือความผิดปกติในอวัยวะต่าง ๆ ได้อย่างละเอียด และการตัดชิ้นเนื้อยืนยันผลย่อมสร้างความแม่นยำในการวางแผนการรักษาได้ดียิ่งขึ้น การป้องกันโรคมะเร็งตับรวมถึงการฉีดวัคซีนป้องกันไวรัสตับอักเสบบี การลดและเลิกดื่มแอลกอฮอล์ รวมทั้งการดูแลสุขภาพตับด้วยการหลีกเลี่ยงสารก่อมะเร็งในอาหาร ส่วนมะเร็งท่อน้ำดีนั้น การหลีกเลี่ยงอาหารดิบที่อาจปนเปื้อนพยาธิใบไม้ในตับถือว่าสำคัญ และการรับประทานอาหารที่ปรุงสุกอย่างปลอดภัยควบคู่ไปกับการตรวจสุขภาพประจำปีและคัดกรองโรคอย่างสม่ำเสมอเป็นกุญแจสำคัญในการจับโรคตั้งแต่ระยะแรก ผู้ป่วยที่พบอาการผิดปกติ เช่น ปวดท้องขวาบน ตัวเหลือง น้ำหนักลด เบื่ออาหาร อาจต้องได้รับการประเมินจากแพทย์อย่างเร่งด่วน เนื่องจากอาการดังกล่าวเป็นสัญญาณของโรคที่อาจอยู่ในระยะลุกลามแล้ว การรับรู้และศึกษาข้อมูลเกี่ยวกับมะเร็งเหล่านี้จึงช่วยเพิ่มโอกาสในการรักษาให้ได้ผลดีและยืดอายุขัยได้มากขึ้น การให้ความรู้แก่ประชาชนเกี่ยวกับปัจจัยเสี่ยง อาการเบื้องต้น และวิธีการป้องกันมีความสำคัญอย่างยิ่ง อีกทั้งยังต้องสนับสนุนการเข้าถึงบริการตรวจคัดกรองและรักษาเพื่อลดภาระโรคในระดับประชากรและเพิ่มคุณภาพชีวิตของผู้ป่วยด้วย
