What’s Actually a ‘Safe’ Med-Surg Ratio?
always hear people say that 4 patients is the “safe” med-surg ratio… but honestly, some nights even four feels like too many. It totally depends on the shift. If I’ve got a fresh post-op, a confused fall risk, someone tanking their BP, and another patient who needs constant pain meds, four feels like ten. And what makes it worse? A lot of hospitals don’t stop at four. They push us to five, and sometimes even six or seven like it’s no big deal. I’ve had shifts where I’m sprinting nonstop, trying to balance meds, vitals, admissions, charting, and call lights, and it feels physically impossible to give everyone the care they deserve. So when people say “four is safe,” I kind of laugh… because for a lot of us, four would feel like a luxury.

























































































































Most hospitals I have worked at assigned 6-7 patients on Med-Surg. It can be too much when you may have 2 fresh post-ops, one dementia patient and administering blood to another.
