Next up, let's talk about how we classify the depth of a burn. We can do this based on how many layers of skin are involved:

1️⃣ - Skin is intact! ONLY the epidermis is involved (ONE layer!) - these burns appear pink or red and are often painful.

2️⃣ - Both the epidermis and dermis are involved (TWO layers!) - these burns appear shiny, moist, and red with blisters and edema. These burns are often VERY painful!

3️⃣ - The epidermis, dermis, and hypodermis are all involved (THREE layers!) - these burns appear dry and leathery and can be red, tan, or black. They are usually less painful as a result of damage to the nerves.

4️⃣ - ALL the way down to 💪 muscles and 🦴 bones - these burns appear black and charred with eschar (dry, dead tissue). Clients with this type of burn will typically lose sensation in the area due to the extensive damage.

For example, partial-thickness burns with blisters require careful monitoring for infection, while full-thickness burns necessitate surgical evaluation. Understanding these distinctions is especially useful for nursing students preparing for the NCLEX exam, as burn care questions frequently appear. Moreover, burn depth assessment is complemented by evaluating burn size and patient factors such as age and comorbidities to determine fluid resuscitation needs and potential complications. By thoroughly recognizing these burn characteristics—red or pink color with intact nerve endings in superficial burns, shiny moist appearance with edema in partial-thickness, dry leathery texture in full-thickness, and black charred eschar in fourth-degree—healthcare providers can improve patient outcomes through timely, appropriate interventions and education.
