What is pitting edema?

For starters, edema is a fancy word for swelling. Pitting edema is a build up of fluid (aka fluid retention) in the tissues, and when pressed, it leaves a pit in the skin.

Pitting edema is rated on a scale from 0 to +4. 0 is what regular skin does when pressed (shown on myself in the video). Depending on the time it takes for the skin to “bounce back”, you rate it from +1 to +4.

This can happen for several minor reasons; allergies, too much salt, standing for too long (blood pools), being pregnant, being in the sun for long periods, and even certain medications. However, it can be a sign of something more serious such as heart failure, diabetes, infection, a blood clot, and more. Ps: this is not an exhaustive list, but just to offer some quick insight.

The best course of action would be to see a doctor to ensure it isn’t anything serious. Often times, you’re prescribed a “water pill” (diuretic), which in the most simplest explanation, will make you pee out the excess fluid.

Compression stockings can also reduce fluid buildup in the lower legs, especially if venous insufficiency contributes to the swelling. I learned that while common causes like allergies and pregnancy-related edema tend to be manageable, persistent or severe pitting edema could hint at underlying heart, kidney, or liver issues. This underscores the importance of comprehensive medical evaluation. Regarding treatment, diuretics (“water pills”) are frequently prescribed to help eliminate excess fluid. It’s crucial to follow the doctor’s guidance on dosage and duration to avoid dehydration or electrolyte imbalances. Lastly, paying attention to accompanying symptoms such as shortness of breath, chest pain, or sudden swelling in one leg can be lifesaving. These may indicate serious conditions like heart failure or deep vein thrombosis requiring urgent care. Overall, understanding the causes and knowing when to seek medical advice empower you to manage pitting edema effectively and improve your quality of life.
