Understanding the distinct signs and symptoms of left side and right side heart failure is crucial for timely diagnosis and management. Left-sided heart failure primarily affects the lungs due to fluid backing up into the pulmonary circulation. Key symptoms include dyspnea (shortness of breath), orthopnea (difficulty breathing when lying flat), nocturnal paroxysmal dyspnea (sudden nighttime breathlessness), persistent cough, rales or crackles heard through a stethoscope, increased heart rate, and fatigue or weakness. Patients may feel like they are “drowning” due to pulmonary congestion. In contrast, right-sided heart failure leads to systemic venous congestion. This causes fluid accumulation in peripheral tissues, resulting in swelling of the legs (edema), weight gain from fluid retention, enlarged neck veins (jugular venous distention), increased abdominal girth due to ascites, nocturia (frequent urination at night), and lethargy. The swelling and increased pressure in veins are hallmark signs to distinguish right-sided HF. It is also important to differentiate between systolic and diastolic heart failure types. Systolic HF is characterized by a weakened ventricle that cannot effectively pump blood out, typically leading to a reduced ejection fraction (EF) below the normal 50%-70% range. Diastolic HF occurs when the ventricle is stiff and does not fill properly, preserving EF but restricting volume. Awareness of these clinical differences improves patient assessment and enables healthcare providers such as nurses and cardiac specialists to tailor appropriate interventions. Early recognition using symptoms like swelling, dyspnea, and cough can help prevent disease progression and improve quality of life for patients living with heart failure.
