After sharing some basics on phlebotomy tubes, I got so many questions about the nitty-gritty details, especially about specific tests and how to handle them. So, I decided to dive deeper into some common queries I've seen. Let's tackle them together! The All-Important Order of Draw This is HUGE, especially when you're preparing to draw blood for multiple tests in several tubes. Getting the sequence right prevents cross-contamination of additives, which could totally mess up your results. My instructors always drilled into us the standard order of draw to ensure the best and most cost-efficient technique: Blood Cultures (sterile, often yellow/black tops) Light Blue (contains sodium citrate – essential for coagulation studies like PT/PTT; remember, it should be completely filled!) Red, Gold, or Tiger Top (these are primarily for collecting a serum specimen. Plain red tubes can also be used as a discard tube, especially to clear any air or anticoagulant from an IV line before collecting actual samples.) Green (contains heparin, which is for plasma studies) Lavender/Pink (contains EDTA to prevent clotting – crucial for hematology studies like CBCs; must be inverted immediately after collection!) Grey (contains sodium fluoride/potassium oxalate – perfect for glucose and ethanol specimens!) Royal Blue (contains heparin or Na EDTA – specifically for trace element and toxicology studies, designed to contain no contaminating metals.) Yellow (ACD) (contains Acid Citrate Dextrose – used for DNA studies and paternity testing. This inactivates complements.) Serum vs. Whole Blood – What's the Difference? Many of you asked about collecting a serum specimen versus a whole blood specimen. If a whole blood specimen is required for a diagnostic test, it should be collected in a tube that contains an anticoagulant, like Lavender (EDTA) or Green (Heparin) – this ensures the blood won't clot. But for a serum specimen, you need the blood to clot! That's where plain Red tops or Gold/Tiger-top SSTs (Serum Separator Tubes) come in. These are centrifuged to separate the serum after clotting occurs. To Centrifuge or Not to Centrifuge? Ah, the centrifuge! Not all tubes need it. Generally, tubes for whole blood (like Lavender EDTA) or plasma (Green Heparin) don't require centrifugation if you're analyzing the whole blood or plasma directly. But if you're getting a serum specimen (Red, Gold) or needing to separate plasma from cells in a plasma separator tube, then yes, it's centrifuge time! This separates the cellular components from the liquid portion. Mixing Additive Tubes This might seem minor, but proper inversion of an additive tube is crucial! Right after you draw, gently invert the tube the recommended number of times (usually 3-4 for Light Blue, 8-10 for others like Lavender). Don't shake vigorously, as this can cause hemolysis and ruin your sample. Remember what the OCR mentioned about LAVENDER tubes – 'Must be inverted after anticoagulants' to prevent clotting and ensure proper mixing of EDTA. Specific Tube Deep Dives What is EDTA in phlebotomy? It's an anticoagulant found in lavender tubes, chelating calcium to prevent blood from clotting, making it ideal for hematology studies. And the red tubes that can be used as a discard tube often contain... nothing! Or a clot activator (if it's a serum tube). A plain red top is often a good choice for a discard tube because it has no additives that could contaminate subsequent draws. For that specific ethanol specimen query, my go-to is always the grey top tube with sodium fluoride and potassium oxalate. Sodium fluoride helps preserve glucose, making it perfect for both glucose and alcohol levels. It's a lot to remember, but practicing and understanding why each tube is used helps so much! Hope this clears up some confusion and makes your phlebotomy journey a little smoother!
