Cystic Acne

Cystic acne, like other acne types, has a multifactorial cause. It often results from a combination of genetics, hormones, and lifestyle factors.

Unlike normal pimples, cystic acne doesn’t resolve on its own. It occurs when bacteria causes inflammation deep within the skin, forming large cysts filled with fluid. These cysts lack a poppable head and are typically found beneath layers of skin on the face, neck, and back (although cystic acne can appear anywhere on the body). The surface of the skin often shows swollen, red bumps that are extremely painful to touch due to the inflamed cysts underneath.

Stage 1: Inflammation

* The initial stage of cystic acne healing involves the improvement of inflammation. During this phase, you may notice a decrease in redness and swelling around the affected area, along with a reduction in pain and tenderness. To support this healing stage, avoid picking, popping, or squeezing the pimple, as this can worsen inflammation, prolong healing, and increase the risk of infection and scarring. Instead, apply cold or warm compresses to reduce swelling and discomfort. Additionally, use gentle, non-pore-clogging skincare products.

Stage 2: Drainage

* As inflammation subsides, the cystic acne bump begins to shrink in size, indicating that the healing process is progressing. During this stage, you may or may not notice pus coming out of the cyst. Continue to resist the urge to pick, pop, or squeeze the cyst. Spot treatments containing benzoyl peroxide or salicylic acid can help shrink the cyst and prevent bacterial growth. Hydrocolloid patches are particularly effective as they absorb excess fluid (such as pus) and protect the skin, allowing it time to heal.

Stage 3: Wound Healing

In the final stage of cystic acne healing, the wound begins to close. The cyst gradually shrinks, and the skin starts to heal. 

* As the cyst heals, you might notice dryness or, in some cases, a small open wound. During this phase, maintain a gentle skincare routine. Avoid harsh scrubs or aggressive exfoliation, as these can irritate the skin and hinder the healing process. To support your skin during this stage, continue using hydrocolloid patches if your cyst has opened up. These patches will protect the healing skin and expedite the healing process. Apply moisturizer to the intact skin. Moisturizers containing ceramides, hyaluronic acid, or niacinamide help strengthen the skin barrier and enhance skin hydration. Consistently apply sunscreen with at least SPF 30 to shield the skin from UV damage and prevent hyperpigmentation.

Stage 4: Pigmentation

As cystic acne lesions heal, you may notice pigmentation (red and/or brown) left behind. This is a normal part of the healing process and can persist for up to 6 months or even longer. 

Post-inflammatory hyperpigmentation (PIH) = Brown spots 

* Post-inflammatory erythema (PIE) =  Red spots can be bothersome, but remember they’re temporary and will fade over time. To speed up the process, use sunscreen consistently (SPF 30 or higher) as it’s the best way to protect your skin. If you only do one thing, make it sunscreen!

- Topical treatments like tretinoin, vitamin C, and others can help lighten dark spots. Chemical peels, microneedling, and lasers can also help fade resistant pigmentation.

Stage 5: Renewal

As new healthy skin cells replace the damaged ones, your skin’s overall texture, color, and appearance will continue to improve. To support this stage of healing, focus on maintaining a healthy lifestyle that includes:

* - A balanced diet rich in fruits, vegetables, and whole grains

- - Regular exercise to promote circulation and reduce stress

- - Avoiding toxins like smoking, alcohol, and excessive sun exposure

- - Adequate sleep to allow your body and skin to repair and regenerate

- - Consistent use of a gentle, nourishing skincare routine

When the cyst started to drain, I found hydrocolloid patches invaluable—they absorbed pus while protecting the skin from bacteria and prevented me from touching the sensitive area. Using spot treatments with benzoyl peroxide occasionally helped shrink the cyst, but moderation was key to avoid dryness. Once the wound started healing, moisturizing consistently with products containing ceramides and niacinamide dramatically improved my skin barrier function and prevented excessive drying or irritation. Sunscreen became a non-negotiable part of my routine, especially to reduce post-inflammatory pigmentation. I learned that protecting healing skin from UV exposure greatly limits lingering red or brown spots. To aid long-term skin renewal, adopting a balanced diet rich in antioxidants, staying hydrated, and managing stress through regular exercise made a noticeable difference. Sufficient sleep was also crucial for giving my skin time to repair. While topical treatments like vitamin C serums helped brighten my complexion, I avoided over-exfoliation to not disrupt the healing process. Ultimately, cystic acne requires a holistic approach that respects each healing stage and supports the skin’s natural recovery. With consistent, gentle care and lifestyle adjustments, it is possible to reduce flare-ups and visibly improve skin texture and tone over time.
