Should you Moonlight in Residency? 🌙

Moonlighting- some programs offer opportunity to moonlight, which is essentially working extra hours as a " junior attending" to be able to get paid more money during residency. YES it sucks that you need to work more to make a decent amount of money BUT it serves as an amazing opportunity to get more experience AND more pay.

Should you do it? It depends- you can't moonlight if your work hours go over 80 hours a week, so some people may be limited because residency already has an overwhelming number of hours of work. If you can do it, I'm ALL for it. Will make another post about the different options ! YAY for more 💰

My last post touched on "MOONLIGHTING WHAT IS IT??" and if you "SHOULD YOU DO IT?", but there's so much more to unpack, especially when it comes to "WORKING 'OVERTIME' TO MAKE MORE $$ IN RESIDENCY" and the reality of "MORE MONEY MORE WORK." What Kinds of Moonlighting Are There? It's not just about acting as a "junior attending" in your own hospital. Moonlighting opportunities vary widely. Internal moonlighting typically involves picking up extra shifts within your own hospital system, often in areas like the emergency department, urgent care, or as a hospitalist on a non-teaching service. This is usually the easiest to get approved and manage. External moonlighting opens up more options, like working at a different hospital, in a private urgent care clinic, participating in telehealth, or even non-clinical roles like medical writing or consulting. Each type offers different pay scales and demands, so understanding your options is key. How Do You Find These Gigs? Finding moonlighting opportunities often starts with your own program. Many programs have an internal system or a list of approved sites. Talk to your chief residents or program director – they often know where residents have successfully moonlighted before. Networking with attendings and other residents can also uncover hidden gems. Sometimes, local hospital systems or urgent care chains actively recruit residents for per diem shifts. Just remember to always get program approval before committing! Navigating the Rules & Regulations This is crucial! The ACGME (Accreditation Council for Graduate Medical Education) has strict rules, especially the 80-hour work week limit. Any moonlighting hours count towards this limit. Your program must approve your moonlighting activities to ensure patient safety and your well-being. Failing to report or get approval can lead to serious consequences for your training. It’s a fine line between gaining more experience and pushing yourself too far, so transparency with your program is vital. The Real Talk on "More Money" Yes, moonlighting can definitely boost your income during residency, which is a huge motivator for many. Hourly rates vary significantly based on specialty, location, and type of work, but they are generally much higher than your resident salary. However, don't forget about taxes! That extra income will be taxed, so factor that into your calculations. While it's tempting to chase every dollar, remember the trade-off. Beyond the Paycheck: The Experience Factor Many residents moonlight not just for money, but for the invaluable experience. Working in a different setting, managing patients independently, or taking on different responsibilities can significantly accelerate your learning and skill development. It allows you to practice decision-making without the safety net of your primary team, helping you grow into a more confident and competent physician. It can even help you explore potential fellowship interests or different practice models post-residency. Is It Worth the Burnout Risk? Staying Sane This is where the "SHOULD YOU DO IT?" question gets tough. Residency is already incredibly demanding. Adding more shifts, even for great pay and experience, can lead to burnout, fatigue, and impact your personal life and studies. I've often heard residents say, "MORE MONEY MORE WORK," but at what cost? It's essential to honestly assess your current workload, energy levels, and personal commitments. If you're consistently hitting close to your 80-hour limit with your regular residency duties, moonlighting might not be sustainable or safe. Prioritize your well-being and patient safety above all else. Listen to your body and don't feel pressured to do it if it's not the right fit for you.
