It’s crucial to pay attention to changes from their baseline rather than focusing solely on the numbers. I learned that what seems like just tiredness can be a sign of a serious change in condition, such as increased confusion or decreased responsiveness. Family interactions are another key area where honesty and professionalism can make a big difference. I’ve experienced how families remember everything you say, so avoiding promises you can't keep helps build trust and manage expectations realistically. One unexpected lesson was that falls often occur when nurses are overwhelmed—during admissions or when call lights are continuously going off. Staying caught up with tasks and being vigilant during busy periods can be lifesaving. Also, documenting patient care as you go instead of waiting until the end of your shift has helped me retain crucial details and provide accurate records. These lessons have shaped the way I approach LTC nursing, emphasizing attentive care, clear communication, and thorough documentation.
