🧠 Trauma-Informed Care Starts With Language

Words matter.

Especially when someone is in a vulnerable position, literally and emotionally.

For many people, pelvic exams can bring up past tru@ma, medical anxiety, or feelings of powerlessness. The way we speak during these moments isn’t just about being “nice” it’s about being intentional, respectful, and safe.

Here’s what trauma-informed language can look like during a speculum or pelvic exam:

💬 Instead of “Open your legs” ➡️ Try: “Let your knees fall gently to the side.”

💬 Instead of “Hop on the bed” ➡️ Say: “Let’s have you get comfortable on the table.”

💬 Instead of “This might hurt” ➡️ Say: “You might feel some pressure, tell me if anything is uncomfortable or hurts.”

💬 Instead of rushing in silence ➡️ Say: “I’ll walk you through each step, and you’re in control. If you want to pause, just say so.”

🩺 Trauma-informed care isn’t a script, it’s a mindset. It means:

✔️ Asking for consent every step of the way

✔️ Offering choices where possible

✔️ Validating discomfort instead of minimizing it

✔️ Centering the patient’s autonomy at all times

Because for someone who has experienced trauma (and that’s more people than you might think), a pelvic exam can feel like a replay of powerlessness. Our job as providers is to rewrite that script with safety, dignity, and care.

Let’s make the exam room a place of trust, not trauma. 💜

... Read morePelvic exams can be a source of significant anxiety and discomfort for many individuals, particularly for those with histories of trauma or medical-related fear. Trauma-informed care reshapes these clinical encounters by prioritizing psychological safety, respect, and patient autonomy, ensuring that language and behavior during exams foster trust rather than retraumatization. Language is a powerful tool in shaping patient experience. For example, instead of command-based phrases like "Put your feet in the stirrups," providers can use gentle invitations such as "May I help you place your feet on the supports?" This shift respects the patient’s control over their body and reduces feelings of vulnerability. Similarly, phrases like "Are you ready for the speculum now?" prepare the patient mentally and emotionally for each step. Offering explanations throughout the exam helps demystify procedures, reduces fear, and validates patient feelings. Replacing "This might hurt" with "You might feel some pressure; please tell me if anything is uncomfortable or painful" invites active communication and reassurance. Providers who continuously ask for consent before each action and offer options empower patients, fostering a collaborative atmosphere. Furthermore, positive affirmations such as "Everything looks good" or "Your health is normal and healthy" can alleviate anxiety post-exam and promote confidence in one’s reproductive health. Being mindful of body language and tone complements verbal communication, enhancing the overall experience. Incorporating trauma-informed language requires a mindset change rather than rigid scripting. Health professionals must remain flexible, patient-centered, and compassionate to effectively support the emotional and physical well-being of their patients. Ultimately, trauma-informed pelvic exams contribute to reproductive justice by acknowledging and addressing individual needs, ensuring equitable, dignified care for all. These principles not only improve patient satisfaction but also encourage regular participation in essential gynecological care, promoting long-term health outcomes.
