Over-the-counter options like ibuprofen and acetaminophen can offer relief for mild migraines or occasional attacks, but they often fall short for moderate to severe episodes and carry the risk of medication-overuse headaches when used frequently—generally more than 10 to 15 days per month. Combination medications like Excedrin Migraine, which includes acetaminophen, aspirin, and caffeine, work well for some by targeting symptoms early, but the caffeine content can also contribute to rebound headaches if overused. Therefore, moderation and timing are key. Prescription options such as triptans (e.g., sumatriptan) and newer migraine-specific therapies like ubrogepant (Ubrelvy) and zavegepant nasal spray have shown higher efficacy by targeting migraine pathways directly. Sumatriptan, especially, is considered very effective but is unsuitable for individuals with certain cardiovascular conditions. The intranasal formulations can be particularly beneficial if nausea or vomiting accompanies the attack, offering rapid absorption when oral medications fail. One challenge with newer medications is access and insurance coverage, which can pose barriers due to cost. It’s also important to approach these therapies under medical supervision to mitigate risks and ensure appropriateness. In my experience, combining lifestyle adjustments—like stress management, sleep hygiene, and dietary monitoring—with appropriately chosen medication improves long-term outcomes. Communicating openly with your neurologist or headache specialist about your migraine patterns and treatment responses enables more tailored care, reducing the chances of medication overuse and improving quality of life.
